Online Paperwork Instructions and Preparticipation Exam (athletic physical) Instructions 2020-2021
Please go to whitehouseisd.rankonesport.com website. You can also access the website through a link on the Whitehouse ISD Athletic Page. Log in using your child’s Student ID number (the last 6 digits). Read the instructions provided on the website to fill out the online paperwork. These forms along with a preparticipation physical are required by the UIL (University Interscholastic League) and Whitehouse I.S.D., and must be completed and submitted before your child is allowed to participate in athletics for the 2020-2021 athletic school year. The following online forms will need to be filled out and submitted annually:
All 7th and 8th grade students involved in the Whitehouse Junior High School Athletic Program will have to submit all online forms before being allowed to participate. A hard copy of a valid athletic physical must be on file with the district athletic trainer in order to be eligible to participate.
Preparticipation Exams (athletic physical)
All incoming 7th graders wanting to tryout/practice/compete in junior high athletics must have a valid athletic physical on file with the district athletic trainers. Physical exams must be performed and dated April 1st, 2020 or latter to be valid
Most 8th graders who participated in school athletics their 7th grade school year should already have a valid physical on file with the district athletic trainer. It is the responsibility of the parent/guardian to make sure the physical is valid. If you are new to Whitehouse or new to 8th grade athletics, a new physical may be required.
Turning in a Hardcopy of the athletic physical
There are a couple of ways to get a hardcopy to the school. It is strongly recommended that a preparticipation physical be turned in before school starts. It is also recommended that a copy is kept at home, just in case.
