Magnus Health FAQ


How do I login to Magnus?

Access your Magnus Health SMR account by logging into your Veracross Parent Portal and clicking the Health Information Button and you will automatically be logged in. 

What information do I need to input into Magnus?

Parents must finish all sections in the Magnus Health Portal, either by filling in forms in Magnus, or by downloading, filling out, and then uploading (or faxing) forms to Magnus.

Parents must:

  • Answer the Conditional Questions.
  • Fill out their child’s Vital Health Record
  • NOTE: The Health Emergency Contacts can only be completed by updating your child’s Emergency Contacts in their Online Profile in your Parent Portal.
  • DOWNLOAD the Annual Physical Form from the Annual Physical Form section of Magnus if due. Have your family physician complete the form, and UPLOAD it to the Annual Physical Form section IF NOT ALREADY COMPLETE.
  • In addition, UPLOAD a copy of your child’s Immunization Form into the Immunization Form Requirement section.  For current students, this may already have been loaded for you. Please check that your child is in compliance.
  • DOWNLOAD and complete the Prescription and Over-the-Counter Medication form, which requires the signature of a healthcare provider, if applicable to your child.
  • DOWNLOAD and complete Action Plan Forms as applicable to your child.
  • Asthma Action Plan
  • Food Allergy Action Plan
  • Diabetes Action Plan
  • Seizure Action Plan
  • DOWNLOAD and complete the Dental Health Report, Athletic Forms, and Independent Medication Use and Carry forms, as applicable to your child.
  • DOWNLOAD and completed the Confidential Health Concerns Form and the Health History Form.
  • Fill out on line the NSHA HS Immunization Compliance Agreement and the Medical release form.

For children who do have medical needs, please make sure your Action Plan is completed, with a physician’s signature, along with any needed Prescription Medication Consent forms.

What do I need to make sure is included on the NYC DOH Annual Physical Form?

Your child needs an annual physical exam,  which includes your child's height, weight, blood pressure, and results of both vision and hearing exams, to be performed by a licensed practitioner, no later than a year from their previous exam.  Newly-dated printouts of information from old exams are not acceptable.

By when do I need to input information into Magnus?

All sections must be completed and up-to-date by July 15, 2023.

Can I give any medical information directly to the Admin team or my child's teachers?

No. All information, including forms from family physicians, should be input directly into the Magnus system. Please do not give anything to teachers or the admin team.

How is over-the-counter medicine administered?

No medication, including over-the-counter medication, can be given without parental AND healthcare provider consent.

  • Over-the-counter medication will be dispensed at the discretion of Smith Street Staff.
  • The amount of medication dispensed is based on your child’s age and weight.

All medication is to be dispensed in the office. Please do not give your child medication to be taken during the day—including cough drops.

ALWAYS UPDATE!

Make sure to update Emergency Contacts as necessary throughout the school year.

EPIPENS, INHALERS, AND OTHER MEDICATION

Please have Epipens, asthma inhalers, and other medication needed in school available to the School on the first day of school.
