Dear Guardians of Incoming 12th Grade Students,

 

Health forms for school registration are due before your child starts their first day at Mundelein High School. Students who do not have this information submitted will be excluded from school per Illinois law.

12th grade students must provide proof of the second dose of meningococcal vaccination (MCV4) given on or after the student’s 16th birthday.

Only one dose is required if the first dose was received at 16 years of age or older.

Please check with your health provider’s office or the Lake County Health Department for the scheduling of their services and make plans to turn in the required forms as early as possible this summer. All school health forms must be be delivered in one of the following ways before the first day of school:

Families may upload their own health forms if they have them during the online registration process.
