How to Complete a W-9
If you are receiving funds as an individual:
- Please fill out Line 1
- DO NOT fill out Line 2
- For Box 3a, check the box that describes you or the legal status of your business NOTE: Vendor must check box based on their IRS tax classification
- If you are filling the form out for yourself and you are not being contracted through a company that you own or work for, check the “individual” box. Name must match social security card.
- If you are a resident alien, sole proprietor, or disregarded entity, type or write your Taxpayer Identification Number
- DO NOT fill out the EIN boxes
- Sign and date the bottom. This must be within the last 365 days.
If you are receiving funds as a business:
- Fill out Line 1 with your legal business name
- Fill out Line 2 if you have a DBA
- For Box 3, check the box that describes you or the legal status of your business NOTE: Vendor must check box based on their IRS tax classification
- For all other entities including LLCs, corporations, partnerships: Name must match entity’s Federal tax documents.l
- 1099 Reportable Classifications: - Individual, Sole Proprietor, Partnership, Trust/Estate, LLC filing as a Sole Proprietor, LLC filing as a Partner.
- 1099 Non Reportable Classifications: - All forms of Government, Employee, Other-Non-Profit, Church/Religious Org, Foreign Business Entity, Corporations, LLC Filing as a Corp or S Corp.
- Nonprofit or 501c3: check “Other” and write in organization type next to it
For all forms, fill out lines 3, 5, and 6, and please remember to check the appropriate box, sign and date. Date must be within the last 365 days. A digital signature is preferred. The state requires new W-9s annually and when business addresses change. Payment is dependent on your timely completion of this form.
If you have a TIN/EIN/SSN but cannot remember it, you can call the IRS at (800 829-1040) of the Social Security Administration at (800-772-1213)
