Dual Diagnosis Worksheet: ADHD & Autism

  1. Identify the negative thoughts that you have about yourself regarding your abilities of ADHD & Autism (e.g. I cannot focus on anything)

  1. Think of emotions related to these thoughts and write how they make you feel (e.g. Frustration)

  1. Reframe your thoughts using a more positive and empowering outlook.

  1. Write down your strengths or positive aspects linked with ADHD & Autism.

  1. Choose an area you want to strengthen. Set a specific and achievable goal to work towards that area.

  1. Write down the steps you will take to achieve your goals.
