Please read and initial each section and sign below. If the participant is under 18 years of age, a parent or guardian must initial and sign this release of liability on their behalf.

The participant agrees to release liability and hold harmless the Greek Orthodox Metropolis of Atlanta and these affiliated entities:

The Diakonia Retreat Center a.k.a. DRC

The Liturgical Arts Academy   a.k.a. TLAA

The Orthodox Christian Academy of Atlanta a.k.a. OCAA

from liability for injuries, illnesses, death or damages that may occur during their time on campus. The participant acknowledges that they are assuming the risks of the retreat/camp activities, and the participant is at their own risk.

The Participant acknowledges that they are in the correct physical and mental health to attend this retreat/camp and participate in the activities offered.

The participant releases the above listed entities from any liability that may arise from the retreat/camp, including negligence or carelessness.

Adults, together with the assistance of their group coordinator, are responsible for contacting any licensed physician, hospital, medical clinic, or emergency medical service organization if need arises. Diakonia Retreat Center does not offer medical services. The DRC suggests that each group has at least one person accompanying them that is certified in CPR and First Aid.

If any provisions of the release of liability are found to be void or unenforceable, the other provisions remain valid and enforceable.

On occasion, DRC & TLAA may take photos during events and activities. We may use these photos for promotional purposes including social media outlets. If you do not want your photo taken and shared, please notify the TLAA director in writing. The Event Director is responsible for notifying the staff at Diakonia Retreat Center.
