Chromebook/iPad Take-Home Participation Agreement
Definitions:
- P-CCS means Plymouth-Canton Community Schools.
- Device means the laptop, tablet, phone or other device, the charger, and any accessories provided by P-CCS.
- Student/Parent means the P-CCS student and parent(s)/guardian(s) to whom the Device is assigned.
- School means any location on P-CCS property or under P-CCS authority, as well as any other private or public location where P-CCS-related work is being conducted.
- Policy means all applicable laws, regulations, P-CCS Board policies, and P-CCS Administrative guidelines, including the P-CCS Technology Acceptable Use policy.
Terms and Conditions:
- The Device is and will remain the property of P-CCS.
- The Device is intended only for use by Students.
- The Device is assigned to Student for school-related work. Student agrees to use the Device for school-related work as defined by the Policy.
- Student/Parent bears full responsibility for all use of the Device and agrees to ensure that all use abides by the Policy at all times and in all locations.
- There is no expectation of privacy for any use of the Device. P-CCS may monitor any use of the Device, but does not guarantee that any use will be monitored.
- If Student/Parent has met P-CCS requirements related to equipment liability (*see below for options), the Device may be taken home or to other locations by Student. However, the Device should be available at the School whenever needed to support School-related tasks.
- Student/Parent agrees to take reasonable precautions to keep the Device safe and in working order. If the Device is lost, stolen, or damaged, Student/Parent agrees to promptly report the incident to the P-CCS Technology Department and law enforcement authorities as appropriate.
- P-CCS will not assist Student in connecting the Device to non-P-CCS networks.
- Student agrees to promptly return the Device to P-CCS when requested. Failure to return the Device is subject to the Policy for P-CCS property.
Types of Devices Issued to Students
To the best of our ability, the following types of devices will be issued to students participating in the take-home program:
- Students in grades K-2 who participate in the take-home program will be issued iPads.
- Students in grades 3-5 who participate in the take-home program will be issued touchscreen Chromebooks.
- Students in grades 6-12 who participate in the take-home program will be issued non-touchscreen Chromebooks.
Participation Requirements:
- Resolve any/all outstanding technology-related debts. [This requirement is being waived for the first device issued to a student in the 20/21 school year.]
- Review and complete this form online, P-CCS Chromebook/iPad Take-Home Participation Agreement in Parent Portal (http://links.pccsk12.com/PP) for each student who is to participate. - Choose ONLY ONE of the following liability options for a Chromebook/iPad:
- Insurance Premium Purchase: Non-refundable payment in the amount of $25 made through RevTrak at http://links.pccsk12.com/CBpremium. This needs to be paid each school year.
- Please note, there is a sliding scale for insurance premiums for families with more than one student participating in device take-home. First child - full premium ($25), second child - 65% premium ($16.25), additional children - free. Deductibles are still charged at the same per-incident rate.
- Insurance Premium Reduced/Waived due to income eligibility: The P-CCS Board of Education has endorsed a policy that waives the device insurance premiums for families eligible for free priced meals with the National School Lunch Program, and reduces the device insurance premiums for families eligible for reduced priced meals with the National School Lunch Program.
- For the upcoming school year, the insurance premium cost for students eligible for reduced-priced meals is $5 for the family’s first student participant, $2.50 for the family’s second student participant and free for additional students participating in the program. For participating students that are approved for reduced lunch, please select the appropriate payment option at http://links.pccsk12.com/reducedCBpremium.
- For participating students who have been approved for free lunch, please “purchase” their free premium at http://links.pccsk12.com/waivedCBpremium
- In the event of accidental damage, theft or loss, there would also be a required deductible to cover the cost of repair/replacement. Student/Parent accepts responsibility for any/all insurance deductibles. In the event of accidental device damage or police-acknowledged theft, a deductible of $30 would be paid; in the event of loss with/without police report, a deductible of $100 would be paid. All participating families are also held financially responsible for any damaged or missing accessories. These insurance deductibles are fixed, regardless of lunch status or other circumstances.
- All insurance claims are subject to approval and may be rejected if the damage is determined to be intentional and/or if the device is removed from its always-on case. If the claim is denied, the family is held financially responsible for the full cost of the repair/replacement needed.
- There is a limit of two (2) insurance claims permitted per student per school year. Following a second claim and the payment of its deductible, should a family wish to continue to have a device issued to their student, they are required to transition to the device deposit option.
- Refundable Deposit: $200 deposit made through RevTrak http://links.pccsk12.com/CBdeposit. Should the device/accessories become damaged, lost or stolen, funds up to the entire amount would be deducted from this deposit to address the cost of repair/replacement. Any funds remaining on deposit would be refunded after the device is returned. If the device/accessories are returned completely undamaged, the deposit in its entirety will be refunded.
- If you already have a $200 deposit on file, then there is no need to pay a second deposit.
- Waiver Request: If you cannot purchase insurance at this time, based on your individual financial situation, you may request a temporary waiver of the insurance premium payment through RevTrak at http://links.pccsk12.com/waivedCBpremium. Please note that due to the additional time needed to process, families requesting a temporary waiver may experience a delay in receiving a district-issued device. We regret any inconvenience that this may cause.
- Keep district-issued device(s) in their always-on case.
- Students are issued district Chromebooks/iPads in an always-on case or shell. This provides additional protection for district Chromebooks/iPads, which will significantly reduce the possibility of accidental damage. Taking a Chromebook/iPads out of the always-on case/shell can damage the case/shell and put the Chromebook/iPads at a greater risk of damage.
- District Chromebooks/iPads are expected to remain in their always-on case/shell at all times. Any student who removes his/her Chromebook/iPad from the always-on case/shell will be held responsible for the full cost of any and all repairs necessary to bring the Chromebook/iPad back to reissuable condition. In the case of students with insurance coverage, these students will forfeit their insurance if they choose to remove their Chromebook/iPad from the always-on case/shell.
Please note, throughout the multiyear 1:1 program, many district-issued devices assigned to students are not new and may not have a manufacturer warranty. Students and families have five (5) school days upon receipt of the device to report any pre-existing faults or defects; otherwise, any issues reported or discovered afterwards will be covered by the deposit or insurance policy.
By selecting “Participate in Take-Home Program” in the dropdown menu below, you as parent/guardian are requesting participation in the 1:1 Chromebook program, and acknowledge that you have read and agree to all of the information stated above, as well as the following:
- Depending on the P-CCS equipment liability option chosen, my family is fully responsible for the insurance deductible or the forfeiture of part or all of a deposit to address the cost of repair/replacement should my device become damaged, lost or stolen. We also accept financial responsibility for all lost or damaged device accessories issued (ie. power adapter, shell, case).
- I will not leave my device unattended unless it is locked in a secure place.
- I will protect the device whenever it is moved from one point to another.
- I understand that my device can sometimes be monitored remotely by software installed by the district, but that parent(s) / legal guardian(s) are primarily responsible for monitoring appropriate use of the device outside of school.
