JOB DESCRIPTION FOR LICENSED VOCATIONAL NURSE AND PSYCH TECH
The LVN/LPT will work with individuals with intellectual disabilities by providing nursing and health care services according to the policies and procedures of the facility and within the standards of the Nursing Practice Act of California. The LVN/LPT works under the supervision of the Registered Nurse.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Supervise and monitor clients at all times both inside and outside the facility for safety.
- Follow Universal Precaution Techniques while providing client care. Hand washing techniques are the most important aspect of Universal Precaution Techniques that should be used while providing client care and doing other activities in the facility.
- At the beginning of each shift read SCOMMs and any other communication via a communication book, GERs, and ID notes in the EHR
- Complete assigned nursing related services for each client.
- Administer medications and treatments as scheduled on the medication administration record and measure and document vital signs as ordered by the physician. Document all medications and treatments as done on the MAR.
- Document change of Condition Report (GER) in the HER and follow through with RN/MD orders. Notify QIDP, FM, Executive Director, RN and Physician of the change of condition. In a life-threatening emergency call 911 and then follow the procedures for the GER.
- Transcribe Physician orders as per facility policy and process and implement the orders
- Report all accidents and incidents you observe on the shift that they occur.
- Complete the medication check at the beginning and end of each shift or as ordered.
- Assists in maintaining adequate drug and pharmaceutical stocks to provide continuity of the administration by ordering and receiving medications from the pharmacy.
- Maintains the security and cleanliness of all medication storage areas.
- Follow up on lab / x-ray orders, obtain results and report and file all final reports.
- Assists in the maintenance of client medical records to assure complete, accurate and pertinent information of all health conditions.
- Communicates with facility staff, other department head and consultants to assess and coordinate needs of the clients.
- Provide and document nursing care to clients as needed.
- Assure prevention and/or early identification of client health needs by making frequent client observations, noting health needs and conditions, and initiate reporting/documentation process of medical intervention.
- Complete a monthly nursing summary for each client by the 7th of each month for review by RN.
- Assist in implementation and updating of nursing care plans as needed.
- Assist RN with the monthly review of new Medication Administration Records and routine renewal of Drug and Die Regimen orders.
- Monitor all resident weights, heights and vital signs. Report any significant change to RN.
- Monitor all resident immunization requirements and assist in the coordination of delivery and administration of needed vaccines.
- Assist in the implementation and continuing evaluations of each client’s Active Treatment Plan.
- Assist with client repositioning/transfers as needed.
TRAINING / INSERVICE
- Conduct staff training as assigned by RN
- Complete monthly inservices as assigned
GENERAL
- Respond appropriately in crisis or emergency situations
- Ensure compliance with the state and federal regulatory agency standards and all policies of JonBec Care, Inc.
- Maintain the confidentiality of all client care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
- Report all allegations of client abuse and/or misappropriation of client property and report all grievances made by the client.
- Follow all HIPAA guidelines and regulations, keeping all client information private.
- Perform all assigned tasks in accordance with our established policies and procedures and as instructed by your supervisor.
- Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility.
- Be familiar with all corporate policy manuals kept in the facility.

SPECIFIC REQUIREMENTS
- Must have a valid California Licensed Vocation Nurse license or a Licensed Psychiatric Technician license.
- Must be at least 21 years old and have a valid California driver’s license for a minimum of three years with a clean driving record and must be cleared by company insurance.
- Must be able to read, write, speak, and understand the English language.
- Ability to apply common sense understanding to solve problems and make independent decisions when circumstances require such action.
- Must possess basic computer skill to input data and pull reports in the HER.
- Must possess the ability and the willingness to work harmoniously other personnel.

PHYSICAL AND SENSORY REQUIREMENTS
- Must be able to move intermittently throughout the workday.
- Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum of waist height and carry such weight a minimum distance of length of facility.
- Must be able to complete two person lifts of clients weighing 100 pounds or more.
- Must be able to use a Hoyer lift to transfer clients from bed to wheelchair, or shower chair and back.
- Must be able to bend, kneel, squat intermittently throughout the workday.

In the absence of the RN, the LVN/LPT will be under the supervision of the QIDP.
