Medical Releases: 

Each camper must have a medical release form on file with Calvary Chapel signed by a parent or guardian.

Camp visits and phone calls are not permitted except in an emergency. Homesickness frequently occurs after phone calls and makes it more difficult for the kids. The camp can be reached in an emergency at (928) 445-5225.
