Steps to Obtain Your Business Entity Document (Service Plan) for Ground Ambulance Licensing

1. Access the DOLA Website

• Visit DOLA’s Local Government Information System (no login required).

2. Search for Your Local Government

• Enter your district's legal name or a portion of its name into the search bar.
• If having trouble finding the district, broaden search by selecting a county and local government type.
• Once you find your district’s listing, click “View” on the Filings tab.

3. Take a screenshot of your local government filing

• Ensure the local government name is included

4. Save the screenshot and upload

• Upload to the “Ground Ambulance Licensing Initial or Renewal” application in OATH.
• If you cannot find your district, contact DOLA representatives for assistance.
