Navigating the Department of Social Services (DSS) process can feel overwhelming when you are starting out. You do not have to walk this road alone. Families Count is a branch of the re|planted ministry hosted at Grace Church of Southern Pines. We are here to partner with local churches to surround you with community, dynamic support, and practical care.

Secure your official case plan, protect your health, and access nutritional support.
- Get Your Case Plan Paperwork

- Action: Meet with your assigned caseworker to obtain a physical copy of your Family Services Agreement (DSS-5240). Ask them to outline the exact milestones required to begin face-to-face family visitation.
- Apply for Medicaid & Food Stamps (FNS)
- Where: Apply online via the NC ePASS portal or walk directly into the Moore County DSS office.
- Action: Securing Adult Medicaid ensures your upcoming substance use recovery programs and medical evaluations are 100% paid for. Food stamps provide immediate stability while you focus on healing.

- Medicaid Non-Emergency Medical Transportation (NEMT)
- Action: As soon as your Medicaid is active, contact your DSS worker to set up Medicaid Transportation. The county will arrange free, direct rides to and from all your clinical therapy and medical appointments.
- Rural General Public (RGP) Bus Subsidies

- Action: Call MCTS at 910-947-3389 and ask for the RGP Token Program. This grants heavily subsidized, scheduled rides for critical baseline needs like grocery shopping, pharmacy trips, and DSS meetings.

Commit to structured, documented treatment to show the court your lifestyle transformation.

- Action: Once you secure a job offer, immediately contact MCTS to transition your rides into a Subscription Service Route. This secures a routine, recurring weekly schedule to pick you up at your doorstep, take you to your job, and bring you home safely.
👨👧 STEP 5: Reconnect & Show Progress
Arrive early for your child and present your documented growth to the judge.
- Attend Every Supervised Visit
- Action: Coordinate closely with your DSS caseworker to use your established MCTS transit options to arrive early to every scheduled visit. Consistency and a positive, loving attitude during these visits are heavily weighted in your Family Reunification Assessment.
- Gather Your Records for Court
- Action: Keep a physical folder of all your clean drug screens, certificates of completion from NCWorks, and attendance logs from your Families Count class. Present this documented proof to the Permanency Planning Action Team and the judge.
