346.  ACCIDENT REPORTS

1.        Purpose

The District provides workers' compensation insurance for its employees.

2.        Guidelines

As such, employees are urged to report immediately to the nurse any accident involving personal injury while in the performance of duty.

If the nurse is unavailable, a report should be made to the principal.

3.        Delegation of         Responsibility

It is the responsibility of the nurse or principal to do the following:

  1. Complete all information required on the compensation form except salary.

  1. Keep one (1) copy to file.

  1. Submit other copies to the Education Center where the form will be completed, checked, and mailed to the insurance carrier.

4.        Contact the District’s workers’ compensation company.
