Daily Self Screening - must be completed every day child attends SACC prior to arrival
Breakfast Form - for children attending before school SACC
only needs to be completed for students in grades 1 - 4 (all kindergartners attend breakfast at no cost. 5th and 6th grade students are dismissed from SACC to ride the shuttle bus to Morristown, breakfast is offered there.)
Permission to Administer Medication Form - if your child will be taking any medication (including over the counter meds) while at SACC, this form must be filled out and returned to the school age care coordinator before we will be allowed to administer medication. Children are not allowed to administer their own meds while at SACC.
Allergy Form - if an allergy was indicated on your child’s enrollment form this form almost must be turned in prior to their first day of attendance. (please use the printed version, not the electronic option)
This form must be submitted once every calendar year. Paper copy available upon request.
Sunscreen Permission Form - filled out yearly, if your child attended 2022 Summer SACC you will not need to submit the form again for the school year
Set Schedule Agreement - due by the 20th of the month before childcare services begin
Set Schedule Change Request Form - must be completed by the 20th of the month prior to the time schedule change is needed

Scheduled Early Release Registration Forms:
Thursday, April 6th - due by March 19th
Thursday, June 1st - due by May 19th
Scheduled Non School Day Registration Forms:
President’s Day (2/20/2023) - due by January 29th
Spring Break (3/20, 3/21, 3/22, 3/23) - due by February 26th
Last Day of School Year SACC (6/2/2023) - due by May 19th
*the minimum number of students must be registered by the deadline for SACC to be open on scheduled early dismissal days and non school days
