Dear Student & Parent/Guardian:

There are a few guidelines before the student is allowed to participate in any sport at Iroquois High School…

First, in order to be eligible, each student must be on current grade level with the required class credits for graduation purposes as set out in the academic requirements by each school. The duties of finding this information will be between the Head Coach and Athletic Director.

Second, in order to participate, a student must have an up-to-date and completed Sports Physical Packet on file with the Athletic Department. Each page contained herein should be read thoroughly and all spaces filled out accordingly before turning in, with a completed sports physical examination. A list of each page and its description is detailed for your convenience.

Page 1 – KHSAA “Parental and Student Consent and Release” – It allows students to participate and insures insurance information.

Page 2 – KHSAA “Emergency Permission” – it allows students to receive medical treatment.

Page 3 & 4 – KHSAA “Physical Examination History” – it insures the health history of each student.

Page 5 –  KHSAA “Covid 19 Questionnaire” – it ensures the Covid 19 history of each student.

Page 6 – KHSAA “Physical Examination” – it ensures the current health status of the student to participate.

Page 7 – KHSAA “Medical Eligibility” – it clears the student to be medically eligible to participate in sports. This form needs an authorized signature, date, location, and phone number of the provider.

Page 8 – JCPS “Sports Video” – it details information about JCPS Athletics.

Page 9 – JCPS “Concussion Signature” – it ensures the acknowledgement of those contents.
