PARENTAL AUTHORIZATION, WAIVER AND RELEASE FOR REMOTE LESSONS
School of Rock is offering remote music lessons to parents and students who cannot or are uncomfortable attending music lessons in person due to the coronavirus. School of Rock may also ask or require its instructors to conduct music lessons remotely. By registering for any remote music lesson with School of Rock, you consent to the following:
In consideration of my child’s receipt of remote music lessons through School of Rock, I, for myself and on behalf of my minor child, and for my or my child’s heirs, personal representatives or assigns, do hereby release and hold harmless School of Rock, LLC and its respective subsidiaries, affiliates, franchisees, officers, agents, employees, and members (collectively, “School of Rock”) from any and all liabilities and damages of whatever nature or origin which my child may sustain during, or as a result of or consequence of his or her participation in School of Rock’s remote music lessons, and I do hereby waive, release and discharge any claim or cause of action against the School of Rock for any and all damages and liabilities which may be sustained by my child in connection with or as a result of participation in School of Rock’s remote music lessons.
I recognize that any remote music lessons will not be monitored or supervised by School of Rock. As such, I acknowledge that my child will be alone with an adult instructor during any remote music lessons. I further acknowledge that School of Rock requests that I supervise my child during the entirety of any School of Rock remote music lessons.
By registering for remote music lessons, I acknowledge that I have carefully read, clearly understand and voluntarily consent to this authorization, waiver and release in consideration for my child’s ability to participate in remote music lessons through the School of Rock. I am aware that this is a release of liability and a contract and I enter into it of my free will.
