District Registration for the 2017-2018 school year will be held at Mauston High School on Tuesday, August 15, from 9:00 am to 7:30 pm.  This is the only day for registration this year!

 

All families are asked to attend District Registration on August 15.  It is important that we have 100% attendance to begin the year. All forms may be downloaded, filled out electronically and printed; or printed and filled out with a pencil or pen. Please bring all completed forms with you to the District Registration Day on Tuesday, August 15.

Families in Transition Brochure 

Who are Families in Transition?  Children and Youth under the age of 21 living in various situations:  Shelter, motel, vehicle, campground, temporary trailer, doubled-up with family or friends, in another type of temporary or inadequate housing or a student living away from their parent or legal guardian.

Bus and Dismissal Information 

One form per student each year.  This form is for ALL students, whether or not they ride the bus.

Free and Reduced Lunch Form  Free and Reduced FAQ  How to Apply  Sharing Information with Other Programs        Meal Prices for 2017-18

This form was mailed home as well. It is important for ALL families to complete this form.  Please refer to the letter that accompanies this form for directions. Cindy Gruen will be at District Registration to answer any questions you have, or you can call 847-5451 extension 6686.  This form will remain on the website throughout the year in the event a family situation changes.  Only one form per family is required.

Media Parents/Student Permission Form 

One form per student each year.  

This form allows students to have their pictures used for educational purposes in the media or social media.

 

Family Educational Rights and Privacy Act (FERPA)

The Family Educational Rights and Privacy Act ( FERPA ), a Federal law, requires the Mauston School District, with certain exceptions, to obtain your written consent prior to disclosure of personally identifiable information from your child’s education records. Please see the form for more information.

 

Responsible Use of Technology and Internet Safety Parent Permission Form

One form per student each year; to be returned to each child’s school.

Field Trip Form- Permission and Notification 

This form can be carried with the teacher on a field trip. One form needs to be completed for each student.

Medication Request/Consent Form 

Medications are to be administered at home whenever possible. If it is necessary for a student to receive medication at school, all appropriate portions of this form must be completed before medication can be given at school. One form for EACH medication is required each year.

 

School District of Mauston Housing Survey 

Under the McKinney-Vento Act, Families in Transition may receive assistance. Please review this form and complete ONE for your family if you can answer “yes” to any of the questions on the form. This form is available throughout the year if your housing situation changes at any time.

 

Volunteer Application 

Any parent who wishes to volunteer in any capacity or accompany their child on a field trip when that option exists, must complete this form. Please consider helping out when you can.  This form may be completed at any time during the school year, but please turn in the completed form a minimum of two weeks prior to the event for which you are volunteering.

 

Home Language Survey 

All students new to the Mauston School District must complete this form only once during the student’s enrollment in the Mauston School District.  Please fill out this form on registration day.

 

Questionnaire for Families with Separate Households 

This form should be completed to provide information regarding a parent in a separate household or if you need to inform the school of any changes in custody or visitation rights during the school year.

We prefer our students travel to and from contests with their teams or groups. If you need to transport your student, please use this form for that particular contest.

Athletic Fees

Beginning in 2008, the Mauston Board of Education approved charging fees for participation in school sports as follows:

High School                $30 per sport                Maximum $60 per student per school year

                         (No Charge for Hockey)

Middle School                $25 per sport                Maximum $50 per student per school year

$150 Maximum per family per school year.  No refunds will be given

If a family qualifies for free and reduced lunch, the athletic fees will be waived if the parent/guardian completes the form titled, “Sharing Information With Other Programs”. The Athletic Fee Remittance form and fee must be completed before a student is allowed to participate in his or her activity. Athletic Director Randy Gyllin will be present at District Registration Day. This form and fee can also be turned in to Cindy Gruen in the District Office.

Activity Fees

Band Instrument is $30 per instrument per school year

HS Parking pass is $30 per vehicle per school year
