Instructions for Activity Coordinators/Planners, Faculty and Presenters to complete the Disclosure of Financial Relationships.
Step 1: Go to umich.cloud-cme.com and sign into your MiCME account. Instructions here if needed.
Step 2: Click on My Tasks from the main menu.
NOTE: If My Tasks does not display in this menu, please reach out to the Activity Coordinator to request being added to the activity or submit a ticket here.
Step 3: Click on Global Tasks.
A Begin button indicates an incomplete task.
An Update button indicates that the task is complete but can be edited as needed.
NOTE: It is recommended that you have all of your financial relationship information with you before starting the disclosure form so you can finish in one session.
Leaving the form or clicking Submit while required fields remain blank may not save entered information.
Step 4: Click on the Begin button, read the informational text, and continue to the questions in the form.
Step 5: Answer the Declaration Question - if Yes, additional information is collected for each company (entity) where a financial relationship exists. If No, continue to the next question.

Step 7: For those who have a relationship as an Owner, Executive or Employee of an Ineligible Company, you will need to attest to the following questions.
Choose Agree or Disagree for each question. If you choose Disagree, the OCME Financial Mitigator will reach out to discuss these selections with you.
Step 8: Attestation - All those who are disclosing relationships must also attest that those disclosures are complete and comply with accreditation guidelines. For each attestation question, choose Yes or No.
Step 9: Electronic signature - A checkbox and pre-populated name field are provided so that you may sign the disclosure electronically.
Step 10: Click the Submit button to submit the form.
(If there is an error and you need to start over, click the Reset button to clear the form.)
