The Root of Anxiety Worksheet

  1.  Think about the situations or the triggers that cause you anxiety. Write the emotional responses associated with the triggers.

  1. Identify the physical responses to your anxiety (e.g. clammy hands, dry mouth, shaky knees).

  1. Write about the first times you felt anxiety. Write what triggered the anxiety.

  1. Think about the repeating patterns in your thoughts and triggers when you feel anxious. Write them down.

  1. Write down beliefs that are related to your triggers (e.g I feel like I'm not enough).

  1. List some coping strategies to tackle your anxiety triggers.
