The Root of Anxiety Worksheet
- Think about the situations or the triggers that cause you anxiety. Write the emotional responses associated with the triggers.

- Identify the physical responses to your anxiety (e.g. clammy hands, dry mouth, shaky knees).
- Write about the first times you felt anxiety. Write what triggered the anxiety.
- Think about the repeating patterns in your thoughts and triggers when you feel anxious. Write them down.
- Write down beliefs that are related to your triggers (e.g I feel like I'm not enough).
- List some coping strategies to tackle your anxiety triggers.
