… am a plaintiff in a Summary Process Action.  Upon obtaining an Execution for Possession, I have hired Constable Michael B. Fixman, Jim Appleyard, a licensed and bonded Public Warehouser, and Jim Granitsas Moving & Storage, movers to obtain possession of the subject premises, subject to the requirements of M.G.L.c.239, s 3 and M.G.L.c.239, s. 4.

I have been quoted the below rates and terms for the services of Jim Appleyard Bonded Storage, Inc. and I acknowledge payment is due when services are rendered.  I agree that additional charge may result if access is unreasonable at mover’s discretion.  I am aware that the rates are for the cost of removing goods to the place of storage only, and that my failure to pay all fees within 30 days will result in additional charge of $250.00.  I further acknowledge all fees and balances outstanding more than 30 days will be charged interest at a rate of 1-½% per month, 18% per annum, and I may be subject to collection expenses including, but not limited to, reasonable attorney fees, if said fees and balances are not paid timely,
