What is the presenting problem? Describe the client’s current problematic behaviors and explain the DSM diagnosis.      

What are the predisposing factors? Consider all factors throughout the client’s lifetime that could have contributed to the development of the problem.

What are the precipitating factors? Why is the client seeking therapy now? What has worsened/triggered his problem?

What are the perpetuating factors? What internal and external factors play a role in maintaining the client’s condition/problematic behaviors?

What are the protective factors? Describe the client’s strengths and qualities that can help him effectively deal with the problem.
