Cell Phone/Head Phone Policy

(Let’s Get our Kids Back and Learning)

  1. No cell from bell to bell (7:45 am-2:00 pm)
  1. Cell phones are not allowed in pockets (not on the students’ body)
  2. Students’ backpacks  are a great place to safely store these devices.
  3. No Cell phones/headphones in the Hallways between classes or at lunch.  Students will be reminded by teachers.
  1. Cell phones and airpods/headphones may be used on school grounds (inside or outside) before school, after school only.
  2. Airpods/headphones need to be put away during the school day.
  3. Cell phones and/or other electronic devices may be permitted for medical reasons. Please communicate these needs with the student’s teachers and the office.
  4. Smart watches are allowed until they become a distraction to the learning environment.
  5. If students need to place an emergency phone call during class, they should request to go to the main office.
  6. Personal laptops and gaming devices are not allowed at school.
  7. The school and its staff are not responsible for any damage to or theft of students' electronic devices. Students must properly secure and take care of their own phones.
  8. If students violate the school’s cell phone policy, the following may occur:

Washington County School District recognizes that every student should have the opportunity to learn in an environment, which is safe, conducive to the learning process, and free from unnecessary disruptions. To foster such an environment the following policy has been adopted and is based on the following principles:

1. Each student is expected to follow accepted rules of conduct. 2. Each student is expected to show respect for other people and obey persons  in authority at the school. 3. The policy applies to students while in the classroom, on school grounds, in school

vehicles, and in school-related activities and events.

The administration and staff of Lava Ridge will do everything possible to help students be appropriate and remain in school. However, at times, it may become necessary to discipline a student. (suspension, court referral etc.) (see entire policy at www.wash.k12.ut.us)

GROUNDS FOR SUSPENSION, EXPULSION and TEMPORARY DISCIPLINARY TRANSFER (TDT) 

1. Illegal Behavior; Frequent or willful disobedience, defiance of proper authority, or disruptive behavior.

2. Use of foul, profane, vulgar, or abusive language. Taunting, Harassment, Bullying or intimidation of other students or staff will not be tolerated for any reasons including, but not limited to, race, gender, religion, mental, physical or emotional illness or disability.

3. Willful destruction of school property. (Vandalism)

4. Behavior or threatened behavior which threatens safety (Bullying, emotional, physical or sexual harassment regardless of where it occurs.) refer to 3.2.1.3 at WCSD website

5. Possession or use of alcohol or tobacco (including E cigarettes).

6. Possession or use of pornography on school property, which includes nude or semi-nude images on personal electronic devices, sent or received by students (“sexting”).

7. Behavior or wearing any article of clothing that signals gang affiliation.

8. Possession, control or actual or threatened use of a real weapon, explosive or flammable material.

9. Actual or threatened use of a look-alike weapon with intent to intimidate another person or to disrupt normal school activities.

10. Sale, control or distribution of a drug, imitation controlled substance or drug paraphernalia.

11. Using force or threatened force which if committed by an adult would be a felony or a class A misdemeanor.

12. Other disruptions such as repeated tardiness/truancy, theft, dishonesty, cheating, gambling, problems with cell phones or other electronic devices or dress code violations.

Suspension is not the deprivation of a right to learning, but is the temporary denial of social interaction  and the removal from the classroom setting. (See WCSD Safe Schools Policy #2110)

BULLYING 

It is bullying if you feel hurt because individuals or groups are:

• Calling you names and/or threatening you, or saying things about

• Pressuring you to give someone money or possessions

• Hitting you, damaging your possessions

• Spreading rumors about you or your family

• Using text, email or web space to write or say hurtful things (cyber-bullying)

It is bullying if you feel hurt because of things said about your ethnic background, religious faith, gender, disability, special educational needs, appearance, or issues in your family.

Washington County schools do not tolerate bullying.

There is a School Board policy (policy #3510 at http://www.washk12.org/) prohibiting any act of bullying. It is the schools responsibility to:

• Make sure that the person being bullied is safe

• Work to stop the bullying from happening again

• Provide support to the person being bullied

• Provide consequences for the bully

What should you do?

Talk to someone you trust (parent, teacher, friend, brother or sister, etc.) and get them to help you take the right steps to stop the bullying. If you feel you are being bullied:

• Stay calm and look as confident as you can

• Be firm and clear — look them in the eye and tell them to stop

• Get away from the situation as quickly as possible

Immediately tell an adult what has happened or, if you do not feel comfortable telling an adult, tell another student or you can text (435) 414-0404 which is the LRI Bully reporting line.

If you have been bullied:

• Tell a teacher or another adult in your school, Tell your family

• If you are scared to tell a teacher or an adult on your own, ask a friend to go with you

• Keep on speaking until someone listens and does something to stop the bullying

• Don't blame yourself for what has happened

If you find it difficult to talk to anyone at school or at home, you can text or talk a message to the school administrators/counselors by following these steps: 1. Go to www.lavaridge.net 2. Select the “Report a Bully” or 3. Text us your message. The number is (435) 414-0404. This message goes to 6 individuals on our leadership team so that we are made aware quickly and respond appropriately.

Hazing means any action taken or situation created that causes, or likely to cause harassment, bodily danger or physical harm, serious mental or emotional harm, extreme embarrassment or ridicule, or personal degradation or loss of dignity to any student or other person associated with the school. The district strictly prohibits students from engaging individually or collectively in any form of hazing or related initiation activity on school property, in conjunction with any school related activity, or involving any person associated with the school regardless of where it occurs. Any student who participates in hazing will face immediate disciplinary action, up to and including suspension, expulsion, exclusion, and loss of participation in extracurricular activities. Students who participate will also be referred to the proper law enforcement authorities.

STUDENT NON-DISCRIMINATION AND PREVENTION OF HARASSMENT In accordance with the Civil Rights Act of 1964, the Washington County School District does not discriminate on the basis of race, color, national origin, sex, disability, gender identity, sexual orientation, religious affiliation, age, pregnancy, parental or marital status, in its programs and activities and provides equal access to the Boy Scouts and other designated youth groups. SEXUAL HARASSMENT - Policy 2116

 Behaviors such as physical or sexual harassment will not be tolerated. This may include

• Derogatory, demeaning, offensive jokes, teasing, or comments of a sexual nature

• Graphic remarks or sexual comments about an individual’s body

• Sexual suggestive or obscene pictures, notes, letters, emails, or objects

• Grabbing, pinching, or touching private areas

• Deliberate shouldering or bumping

• Sexual gestures, or unwanted pats or hugs, and inappropriate touching

• Any form of sexual threat, intimidation, or exploitation

• The student has the right and is encouraged to report the problem immediately to the principal, counselor, or assistant principal at their school. Students should not feel embarrassed, intimidated, or reluctant to file a valid harassment report. (WCSD Policy #1420)

Consequences for Student Sharing Recording of Harassment:    TBA

Any offense may require suspension or expulsion and involvement of School Resource Officer. NOTE: Kissing and holding hands are not acceptable gestures at Lava Ridge Intermediate School. If this action takes place parents may be contacted.

SUSPENSION Suspension is the temporary loss of privilege of attending school. Suspensions may be for a ten- day period for each incident. In such situations parents will be contacted by the school to come and pick their child up from school. The child will become the responsibility of the parent during school hours.

EXPULSION A student who commits a violation involving a real or look alike weapon, explosive or flammable material, or drugs may receive a Temporary Disciplinary Transfer or  expelled from school. A student may be denied admission to school on the basis of having been TDT’d or  expelled from that or any other school.

CRIMINAL TRESPASSING Law Enforcement has the authority to enforce unauthorized use of school property, all trespass & loitering laws, all traffic & parking laws on WCSD property. All persons and vehicles entering school property are subject to search. (WCSD Policy #5220)

POSITIVE BEHAVIORAL INTERVENTION SUPPORT (PBIS) At Lava Ridge we have implemented a PBIS program that includes the following: 1. Set up Behavioral expectations for each area of the school. The Warrior Way includes the following 3 expectations: Be Safe, Be Respectful and Be Responsible. Each area has signs that explain the expectations for the areas. These can be found on page 22.     2. Teachers can scan Gold points to a  students PBIS account that are following the Warrior Way. 3. Reinforce these good behaviors by giving  Gold cards or Gold points to students following the expectations. Students can go to the  Warrior Way-Mart for rewards. 4. If students are struggling to follow the expectations their name will be brought up in the Teacher Interdisciplinary Team meeting. This group of teachers will pull together student information to identify the root cause of the behavior and will discuss a common intervention that will be implemented in every class. Parents are a key part of this process. 5. If the behavior is still occurring after they have tried 3 different interventions for a minimum of 6 weeks then the student will be referred to the Tier 3 intervention team.

ATTENDANCE POLICY (WCSD Policy #2900)

Parents - Please have your students attend school regularly.  This means they need to miss less than 5% of school days in a school year.  We encourage families to keep a calendar and to visually track your student’s attendance.  Many families are shocked when they see how many days their students are absent.  It is important that we get back to a culture of putting school as a high priority that will directly affect their success in the future.  As a community we can collectively work to restore the mindset of the importance of attending school regularly.   There are many variables:  sports, family vacations, anxiety, illness, laziness etc.  We are asking that families plan the majority of their student’s non-school activities outside of the school day.  

Students with Chronic Absenteeism (over 10%) may not qualify for eligibility to extracurricular activities and/or out of school activities.

The Washington County School Board believes that student learning and achievement is directly related to attendance.  WCSD strives to make every effort to be in compliance with state law as well as work proactively together with parents, community resources to encourage the regular attendance of all Washington County students.  Regular attendance is important to success in school and is required by law. When you miss school, you miss out! When a student is absent for any reason a parent must call the school at 652-4742 as soon as possible.

The following is an excerpt from the WCSD Attendance Policy (#2900)  The "State Compulsory Education Requirements" (Utah Code, Section 53A-11-101) directs parents and schools as follows:

* A person having control of a minor who has reached the age of six years, but has not reached the age of eighteen years, shall send the minor to a public or regularly established private school during the school year of the district in which the minor resides.

Utah Code 53A-15-1501 Parental Rights in Education grants that the parent or guardian has the right to reasonable academic accommodations. The Washington County School District recognizes that the parent or guardian is the primary person responsible for the education of their student, and the school is in a secondary and supportive role to the parent or guardian. This includes the parent’s right to excuse the parent’s child from school for valid reasons as per 3.3.

* Students are responsible for the completion of any work missed due to absence, regardless of the reason for the absence (excused, unexcused, truant). Students with absences prior to the midterm week will be allowed to submit completed make-up work through the midterm grading period.  Students with absences following the midterm week will be allowed to submit completed make-up work through the end of the current grading period.  The school is encouraged to provide opportunities for students to make up lost time and assignments; such as: lunchtime or after school make-up.

*  A parent, teacher, or a school administrator may request a conference to discuss the attendance of a student whom they deem to have an excessive number of absences; and follow procedures as per policy.

Excused absence: The school shall record an excused absence for valid reasons, as listed below, when the parent/guardian:

* Submits a written statement at least one school day before a scheduled family event (including an extended absence) or a scheduled proactive visit to a health care provider if the student agrees to make up course work for school days missed for the scheduled absence in accordance with this policy.

* Excuses the student for illness or for a family emergency (such as a family death), both of which may be excused via phone, in person or by note within one week after the absence.

* Illness or accident: A student who is absent for health-related reasons for a period of five (5) or more consecutive days, who is undergoing surgery, or who has been injured in an accident, qualifies for immediate homebound services. School work will be collected for the parent to pick up or an alternative educational program may be developed.

* Chronic illness: Chronic health problems may or may not interfere with attendance at school and learning in the classroom. It is extremely important that parents notify the school concerning health-related problems that their students may have.  Documentation from a health care provider is required in validating the seriousness of the chronic illness. If the chronic illness hinders a student's attendance, the parent is invited to meet with a school administrator to formulate an individual attendance plan that will meet the student's needs. It is the responsibility of the parent to schedule such a meeting.

* Additional valid excused absences include: an approved school activity; an absence as permitted by a school age minor’s Individual Education Plan (IEP) or 504 Plan; or any other excuse established by the school district; such as: office excused (if student is detained in the office by school administration); court/detention absences; or homebound.

* Excused absence status will not be considered with regard to implementing the required interventions; unless the excused absenteeism becomes so excessive that it impacts the student’s ability to progress educationally and/or is affecting the student’s academic performance at which point the school may provide interventions as per policy procedures.

Tardy: A tardy is defined as reporting to class after the bell to start class has sounded.  Tardy rationale:  Tardiness not only hinders a student's ability to profit from instruction at the beginning of class, but it interrupts the work of the other students who made the effort to be on time.  Research is abundantly clear that punctual students perform better in school and at work.  Each school is required to establish a uniform tardy policy for the school – to include consequences. Each school’s tardy policy will be included in the school’s handbook.  All tardies will be recorded on PowerSchool.

It is the responsibility of the student and/or parent(s) to monitor tardies on PowerSchool, through progress reports, and by communication with the teacher.  A tardy may be excused by a parent when an emergency situation results in a student arriving at school late.  For secondary students, a student may not be excused for tardiness between classes by a parent.  Tardies less than 10 minutes (T), Way Late - 20+ minutes late to class (W = way late),  Excused Tardy (L) .  

Tardy Consequences:  5 Tardies = 3 days of LD + Parent Contact; 10 Tardies = 3 days of LD + parent contact; 15 Tardies = 3 days of LD + parent contact;  20 T = 1-2 days of GRIT (ISS)  + Parent/Team Conference.   Tardies reset each quarter, except for those who reach 20 Tardies or higher.  They will now be considered a Tier 3 Tardy concern and will be monitored closely.

REQUEST FOR MISSED HOMEWORK: If a student is absent they are still required to make up missed work. Please check the teacher’s schoology class to access details about missed assignments. If you have questions, please email teachers directly and they will be happy to help you.

TRUANCY

Any student that is out of class without excuse will be considered truant. Students must be checked out through the office by parent/guardian before leaving campus. If a student leaves class or campus without following proper checkout procedures they will be marked truant and face further disciplinary action. When a student is truant they will make up the time by being put on lunch detention. This means that they will go to an assigned classroom and work on missed class work. Each truancy period will require (2) lunch periods.

PARENT NOTIFICATION

• Parents will be notified by email/letter or phone call if their student is having excessive problems with their attendance habits. If progress is not made soon after the school has contacted the family, then it will be necessary to set up an attendance contract with the student. We also have the option to set up a Truancy Mediation Meeting.

APPEALS PROCESS

• Families who feel there were mitigating circumstances that should be considered, are invited to notify in writing to the Vice Principal.

• If one or more absences are not excused an appeal form must be completed and presented to the “Attendance Review Committee”.

• Dissatisfaction with the decision of the committee can be appealed to the Washington County School Board through the Superintendent.

GRIEVANCE PROCEDURE The grievance procedure is to provide a means by which parents may present for administrative resolution, unresolved questions, dissatisfactions, or disputes regarding interpretation of application of District Policies and procedures. BUS TRANSPORTATION

Bus transportation is a privilege granted by the school district at considerable expense. If a student misbehaves on the bus, the driver will file an official complaint. The student will be counseled, warned, and may have his/her bus privileges suspended! Students must do what the driver tells them to do; the driver is in charge of student’s safety!

SCHOOL BUS RULES The driver of the bus is responsible to transport students to and from school safely; therefore, they cannot and will not tolerate any dangerous or distracting behavior on the bus. Students must follow these rules for their safety as well as that of every student on the bus. (WCSD Policy #7100) 1. The driver is the person in charge. You will follow his or her instructions at all times. 2. If seating is assigned, accept it immediately. 3. Courteous behavior is required while waiting in line and on the bus. 4. All body parts must remain in the bus. 5. Sit flat facing the front. 6. Maintain a low voice level at ALL times. 7. No food or drink may be consumed on the bus. You may carry a sack lunch. 8. When the red lights of the bus are flashing, look for the driver to signal it’s OK to cross the street. Look both ways, and then cross. 9. Never go into the danger zone of the bus! Remember, students who do not follow these rules, forfeit the privilege of the bus.

Note: All bus students will be issued a bus pass that assigns them to a specific bus stop.

The students will only be allowed to get on and off at the assigned stop. If friends are planning to go home after school with a student, parents will need to provide transportation. LRI will have a Bus Safety patrol that will empower students to help monitor and report inappropriate behavior on the bus. We are excited to improve the quality and safety of bus transportation here at LRI.

DRESS CODE (WCSD Policy #2200)

Consequences for violation of the Student Dress and Grooming Policy:

A student in violation of the "Student Dress and Grooming Policy" may call a parent to have a change of clothes brought or be sent home to make the changes necessary to be in compliance with policy. The student may return as soon as the required changes are made. Student will also be allowed to change into PE clothes or borrow a school shirt/Shorts from school  if available. A parent meeting will be required if students are repeat offenders. This can be a sensitive topic for some families and we always try to work with those out of compliance with dignity and kindness. Please help your child to follow the dress code so potentially awkward situations can be avoided for your child. Students who dress or groom in a manner that demonstrates gang affiliation may be subject to suspension and/or expulsion. Suspension from school may occur for continued disregard for the Student Dress and Grooming Policy.

GRADING PROCESS

Academic Grade (A,B,C, NP - refer to the LRI grading scale) The academic grade will be given according to results on tests, quizzes, projects, essays, labs.... anything that demonstrates proficiency. An “NP” grade means that the student is not proficient. Teachers will notify parents when their student has an “NP” after two weeks into each quarter. Teachers may change the “NP” grade if proficiency has been demonstrated.

ACADEMIC INTEGRITY Lava Ridge Intermediate School requires a high level of scholarly behavior and academic honesty on the part of students. Violations by students exhibiting dishonesty (cheating) while carrying out academic assignments or tests may result in no credit or other disciplinary action.

REPORT CARDS—MIDTERMS The school year is divided into four quarters. During the year, students will receive four report cards, one each quarter. A progress report will be emailed to parents at mid-term and a report card at the end of the term. Teachers will utilize “Advisory” time to help students have the extra time needed to improve grades and learning.

TECHNOLOGY CERTIFICATION

Washington County School District Acceptable Use Policy #3700 (available at www.washk12.org) Here is the Link:  WCSD Technology Acceptable Use Policy.  This policy  requires that students are instructed in appropriate online behavior including online safety, interacting with other individuals on social networking websites and in chat rooms, and regarding cyber-bullying awareness and response. The Technology Standards adopted by LRI help all students to know how to respond when difficult situations occur while using the internet at school. The student use of personal devices (cell phones, eReaders, iPads, iPods, Tablets, Laptops) that can access the internet are allowed only within the classroom when a teacher allows devices for a specific lesson. Personal devices can only be used by the student who owns the device and is the responsibility of that same student. Personal devices/earbuds/air pods can be used responsibly during lunch time when finished eating or in the morning before school starts. Do not use cell phones, ear buds or airpods  in classes unless given permission from the teacher. The school will not be held responsible for stolen cell phones.  We are encouraging families to not have their children bring cellphones to school. If they chose to bring one, it needs to be stored in their backpack.  Cell phones need to stay in backpacks. Teachers and office staff have the right to collect a student's cell phone if it has become a distraction for the student.

When you are using technology, consider the T.H.I.N.K. questions to help you pass on appropriate information. This is important everywhere, but especially at school we want all students to feel welcome and safe.

HOW DO I GET MY TECHNOLOGY CERTIFICATION?

1. Participate in Advisory class Digital Citizenship lessons.

2. If you abuse technology here at LRI you will lose your technology privileges and be required to take an online technology course and  have a parent contacted.

It is a privilege to use technology so don’t abuse it!!

MISCELLANEOUS ITEMS

ADVISORY TIME Monday - Thursday  we will provide an advisory period class to start the school day for the purpose of hearing announcements, having a Social/Emotional Lesson/mindful minute, Pledge of Allegiance,  re-teaching, work make-up, and enrichment.   This class period allows students and teachers to have the extra time needed for concept recovery without missing any regular class time. Monday is longer for our Social Emotional Learning (SEL) lesson and then priority days for re-teaching are:, Monday - Fine Arts for concert prep, Tuesday–Science, Wednesday- Language Arts, Thursday-Math, Friday - CCA/Social Studies etc.    Students that are caught up and have no missing work may participate in our Gym Advisory Program (GAP).

BELL SCHEDULE The school day at Lava Ridge Intermediate School is from 7:45 a.m. to 2:00 p.m., Monday through Thursday.  Friday is a shortened day that is from 7:45 a.m. to 11:20 a.m.. Students are requested to arrive no earlier than 7:30 a.m. and stay no later than 2:10 p.m. Supervision will only be provided during these stated times.

BUS PASSES All bus students will receive bus passes at the beginning of the school year. District Transportation Department has assigned each student to a specific bus and bus stop according to each student’s address. Should a student move during the school year (within school boundaries) it is the responsibility of the student to alert the office and show proof of new address. At this point the student will be issued a new bus pass. Students will not be allowed on any other bus other than the one they are assigned to. If a student loses their bus pass they can be replaced at the office for $5.00. If a student moves out of the school boundaries during the school year but desires to continue to attend the school, transportation becomes the responsibility of the guardian.

CAFETERIA  Menus are publicized and can be picked up at the office.  All food and beverages are to be kept in designated areas. Clean up after yourself. Please do not crowd or cut in line. Repeat offenders may receive lunch detention. Please, no, yelling, running, or pushing. Students may ask for an application (in the office) to work in the cafeteria. They will be selected and maintained if they have passing grades, good work ethics, and good manners.

CHECK OUT A parent/guardian is the only person who may check out a student from school. A legal guardian must check a student out at the office when leaving the school for any reason. Picture Identification will be required. Students will not be released to anyone other than their guardian unless prior arrangements have been made.

COMPUTERS Lava Ridge Intermediate provides computers for student use in the classroom. In the event that a student alters the software arrangement, physically damages the computer through misuse, or accesses a protected system or program, that student will lose the privilege of using school computers and may face further disciplinary action.  Student carelessness or purposeful damages to school technology will need to be covered by the party responsible.  Also, there is no eating or drinking when using the school computer.

COUNSELING Counseling is available to all students. You can learn more about the services and resources available to students  on our website: counseling.lavaridge.net. Throughout the school year, students will interact with counselors in classroom lessons, small group presentations, and individual counseling for a variety of reasons including social, emotional, and academic supports. The following topics require parental consent and students may bring up these topics unsolicited by the counselor. In such cases, a parent permission form will be sent home with your student. While counselors strive to keep student interactions confidential, parents will always  be notified if the counselor determines that the student is in a dangerous situation (harm to self or harm to others). DISTRACTIONS Every year there seems to be new toys, gadgets  or practices that can rise to the level of becoming a distraction in the school.   We want the students to have fun, but when items or practices start to regularly become distractions or safety concerns then we need to step in. We hold the right to continue to ban objects or activities that distract from the learning environment.

DUE PROCESS If we, at any time act or react in a way that a student truly feels is unjust, we will arrange a private conference during which the student/parent can express to us why he or she feels our actions were not fair. This may or may not change the course of our actions. However, we are always open to calm, rational discussion of any matter.

EARBUDS/Headphones  1. Earbuds/airpods/headphones are not to be worn unless a teacher gives permission in the classroom.  They will be kept in backpacks.  2. Earbuds, airpods or headphones will not be worn in the hallways.  It is important that students hear clearly if there is an emergency or announcement.  3. Earbuds/airpods/headphones will be allowed to be used at lunch.  4. The school will not be held accountable for theft of earbuds/airpods or headphones.  We encourage  earbuds/headphones to be left at home.

ELECTRONIC DEVICES (Student owned) -  No Personal Computers or Gaming systems are to be brought to school.1. Electronic devices (Cell phones)  need to be stored inside student backpacks during class (not outer pockets of backpack, not in clothing pockets) and in the hallways unless the teacher gives permission. If a student is caught on their technology during class or in the hallway without permission teachers may educate them on proper time and use. Teachers have the right to confiscate a phone/technology if students have not complied with the rules. 2. Cell Phones  can NOT  be used during lunch.   We are a 1 to 1 school which means that students do not need to bring their own technology to school as we provide technology for each student. Cell phones, MP3 players, cameras, toys, and other electronic devices frequently distract students from learning. These items are also subject to theft. We will not be responsible for these items. We want to help students learn how to be responsible digital citizens and know when to use technology and when not to.  Repeat offenders will have their phones taken to the office. If it comes to the office for a 2nd time a parent will be required to come and pick it up. Please do not call/Text your child during school hours. If there is a need for communication please call the office and we will pass the message along to your child.  Repeat offenders may receive further consequences.  If students need to use their cell phone for an emergency they may come to the office to make that call/text. ( Please refer to the Earbuds section above).

ELECTRONIC DEVICES (School owned) - Lava Ridge is a 1 to 1 school. This means we have chromebooks for each student in their classes.  These computers are expensive to maintain and repair when needed.  In the event that a student alters the software arrangement, physically damages the computer through misuse, or accesses a protected system or program, that student will lose the privilege of using school computers and may face further disciplinary action.  Students will be held accountable for covering the repair costs of damages due to careless or intentional damage to school technology.  

EMERGENCIES—ILLNESS—MEDICINE If a student becomes ill at school, he/she should come to the office and contact the parent. If medication must be taken during the school day, Washington County School District requires a form to be signed by the parent or guardian. The medication is to be kept in the office and consumed there.

FEE WAIVERS Students may apply for fee waivers by submitting the Fee Waiver Application that is available at the office. Based upon income verification or appropriate documentation your fees may be waived. Please contact the school for more information.

FIELD TRIPS/Activities - Students will need to get parent permission in order to participate in a school sponsored field trip.  Students represent the school and the school Warrior Way Code of conduct is expected during the entirety of the field trip.  Students may lose the privilege of attending a school field trip/Activities (performances) if they have failing grades, excessive absences or have been referred to the office for behavioral concerns. Students need to attend school on the day of performances or concerts unless there are extenuating circumstances. Students with Chronic Absenteeism (over 10%) may not qualify for eligibility to extracurricular activities and/or out of school activities.

GIFTS AT SCHOOL We discourage GIFTS AT SCHOOL. Because of the disruption caused by balloons, flowers, and any other gifts delivered to the school, students will be informed of the gifts arrival, but the gift will remain in the office until the dismissal of the school day.

HALL PASSES  Lava Ridge uses an electronic Hall Pass (SmartPass) that students must use when they leave the classroom.  Students will be trained on the use of this system at the beginning of the school year or when newly enrolled in school.

HALLWAY BEHAVIOR Safety in the hallways is paramount to students having a great day at school. The following safety rules need to be followed: Walk at ALL times; Be Aware: look up and listen; Follow directional signs; Keep hands and feet to yourself; Get drinks/visit restroom between classes; Use positive, clean language; Voice Level at a 2; There is to be no gathering of friends in between classes as there is not enough time.  We are to keep moving in the hallways as there is only 3 minutes between classes.  Warnings will be issued to first time offenders but repeat offenders will be disciplined.  Cell phones are kept in the backpack in the hallways.  This will prevent accidents and damage potential as the hallways are very crowded.  Music will play over school sound system to notify there is one minute prior to the tardy bell.

Honors Certificate  Students will be able to choose if they would like to participate in the Honor’s Certificate Program for their Core Classes.  Each subject area (Language Arts, Math, and Science, etc.) will have the criteria ready for students that are interested.  Those who earn Honors Certificates will be eligible for an end of year celebration.

IMMUNIZATIONS State Law 53A-11-301 requires that all students entering the 7th Grade must provide proof of immunizations for the following:

•         Three (3) Hepatitis B and Two (2) Hepatitis A

•         One (1) Tetanus/Diphtheria (Td) booster

•         One (1) Meningococcal immunization

•         Two (2) Varicella (chicken pox) - If the student is already 13 years old, two (2) Varicella doses are required.

Please note that 7th grade students will not be allowed to attend classes until verification of these immunizations has been brought to the LRIS Front Office.

INSURANCE (Student)  At the beginning of the year, student accident insurance is made available on an optional basis to each student. The form made available is to be sent directly to the company. The school does not carry personal insurance on students. Parents can request information in the  front office for the form and further instructions.

INTERNET POLICY Any use of the internet for illegal, inappropriate, or obscene purposes, or in support of such activities is prohibited.  (Actual form can be requested from the school or go to the Washington County School District website at www.washk12.org to see the entire document.)

Handling Students with Lice:  Handling Students with Lice

Locker Room - There will be random searches of lockers as a part of keeping our school safe.  Locker searches by school officials do not require a reasonable grounds justification. A student locker is property of the school. A student has no expectation of privacy in a locker or its contents.

LOST AND FOUND If you lose a book or personal item, check with the office. The student must assume sole responsibility for loss or damage to any school or personal property. The school will endeavor to protect all properties but is not responsible for them.  

LUNCH DETENTION The Lava Ridge Lunch Detention (LD) program will be utilized as a consequence for excessive tardies, misconduct, and falling behind in their core classwork (lower than a C). There will be two teachers that will take roll daily and manage the two different lunch detention periods.  Repeated placement in Lunch Detention may result in placement to the Restorative Justice Room.

MEDIA CENTER The Media Center is a place for quiet study and reading. It is open before school, and after school for student use. When classes are in session, a student should have a pass from the teacher for that period to be admitted. Fines are assessed for overdue, damaged, or lost books.

MOVING—TRANSFERS If a student is moving from Lava Ridge Intermediate his/her guardian should notify the school before the last day of attendance. A checkout sheet will be sent around with the student on the last day of attendance. All fees and fines must be paid before the checkout is complete.

PHYSICAL EDUCATION Students enrolled in PE are required to dress daily in required PE clothing. No excuse is acceptable unless a written statement from a doctor or teacher contact by the parent indicates otherwise. A locker will be issued to each student.. All personal items of value are recommended to be kept in the locked locker.  Student backpacks will be stored in the gym by their student number.  Camera surveillance is active in the gym. The student assumes all responsibility for loss and/or damage to personal property.  

The Protection of Pupil Rights Amendment (PPRA) PPRA Policy

POSTERS All posters and/or bulletins must be approved by the school administration. The school district office must approve announcements or posters pertaining to commercial ventures. Posters and bulletins are not to be attached to painted walls. Posters and signs may not be placed around the school without administrator approval.

Displays of Affection Students are expected to show good taste and conduct themselves respectfully at all times. Inappropriate public displays of affection will not be tolerated.  (This includes holding hands, kissing, excessive hugging  etc.). This behavior will result in a parent meeting and possible school consequences if inappropriate behavior continues after being warned.  

GRIT Room - (RJ Room)  Lava Ridge has a GRIT  room that will be utilized to help students lacking behavioral skills gain the knowledge and guidance needed to be successfully transitioned back to their regular school schedule..  This room will replace In School Suspension (ISS).  

SEVENTH GRADE GRADUATION ACTIVITY The graduation activity will be for all 7th grade students. If a student has received disciplinary action for severe or repeated behavior throughout the year (including attendance issues (Tardies)) they may lose the opportunity to participate in this activity.

SKATEBOARDS and ROLLERBLADES The riding of skateboards and rollerblades will not be allowed on Lava Ridge Intermediate School Property. Anyone bringing these items to school will be asked to leave them at the office and not bring them again. Should the action continue they will be taken and parents will be expected to pick them up.

Student Leadership Students that are selected to be on one our Student Leadership Teams must be passing all of their classes (determined by summative end of quarter grades) and not be chronically absent.  A student conference will be held if the conditions are not being met and a student may lose their opportunity to serve in that capacity.  A student may also  lose leadership opportunities if they need to be placed in a reading advisory class or if they are not following the Safe School Policy.

SUBSTITUTE TEACHERS Substitute teachers are to be considered and treated as guest faculty members in our school. It is the responsibility of students to see that every courtesy and assistance is offered substitutes. DISRESPECT FOR SUBSTITUTE  TEACHERS IS NOT TOLERATED  and disciplinary action will result.

TELEPHONE USE Students may use the student phone in the office only for school business, illness or an emergency. Students will not be called out of class to take a phone call. Parents may leave a message to be given to the student. Please plan ahead so students will not have to be disrupted during school.

TEXTBOOKS Textbooks are issued through classrooms. Normal book wear is expected during the school year. However, if a textbook is lost or damaged, the student must pay for some or all of the cost depending upon the extent of the damage. Year-end grades and yearbook will not be given until the debt is taken care of.

Truancy (Skipping Class)  If a student skips a class (without parent/school permission) they will get 2 days of lunch detention for each period skipped.  If a student leaves campus and skips a class without permission they will get 4 days of lunch detention for each period skipped.

VISITORS Due to classroom and campus routine and crowded conditions, student visitors are not allowed. Adult visitors must sign in at the office. They will be issued a pass. If you would like to observe in a classroom, please call ahead. Adults will need to show a picture ID to pick up a student.

WELLNESS CENTER The mission of the Wellness Center is to promote student emotional well-being through care and support to students in their pursuit of academic success.   If students are feeling a social or emotional need they can talk to their teacher and request to go to the Wellness Center located in Class A1.

504 Families need to contact the school if they have a student that qualifies for 504 educational services, if they are not already being served with a 504 or IEP plan. If your student has any Doctor diagnosed physical or mental disabilities that need to be considered for receiving an equitable educational experience please contact the school.

IMPORTANT STUDENT AND PARENT RIGHTS FERPA NOTIFICATION

The Family Educational Rights and Privacy Act (FERPA) and District Policy 3260 affords parents and students over 18 years of age ("eligible students") certain rights with respect to the student's education records. These rights are:

1) The right to inspect and review the student's education records within 45 days of the day the School receives a request for access. Parents or eligible students should submit to the School principal or assistant principal a written request that identifies the record(s) they wish to inspect. The School official will make arrangements for access and notify the parent or eligible student of the time and place where the records may be inspected.

2) The right to request the amendment of the student's education records that the parent or

eligible student believes is inaccurate or misleading. Parents or eligible students may ask the School to amend a record that they believe is inaccurate or misleading. Such requests should be in writing and addressed to the school principal or assistant principal. It must clearly identify the part of the record they want changed, and specify why it is inaccurate or misleading. If the School decides not to amend the record as requested by the parent or eligible student, the School will notify the parent or eligible student of the decision and advise them of their right to a hearing regarding the request for amendment. Additional information regarding the hearing procedures will be provided to the parent or eligible student when notified of the right to a hearing.

3) The right to consent to disclosures of personally identifiable information contained in the

student's education records, except to the extent that FERPA authorizes disclosure without consent. The following information may be disclosed without consent:

Directory Information: The following information is designated as student Directory Information: Student’s name, address (to include internet address), phone number, date of birth, grade level, extra- curricular participation, awards or honors, photograph, video or digital images, height and weight (if a member of an athletic team), previous school attended, dates of attendance, and parent’s name. “Directory Information” may be disclosed without prior written consent. The District will also routinely disclose names, addresses, and telephone numbers to military recruiters. Parents or eligible students will have two weeks from the beginning of the school year or date a student enrolls to advise the school district, in writing, of any and all items they refuse to permit the district to designate as directory information for the balance of the school year.

School Officials: School officials with legitimate educational interests may also have

access to records. A school official is a person employed by the School as an administrator, supervisor, instructor, or support staff member such as a bus driver (including health or medical staff and law enforcement unit personnel); a person serving on the School Board; a person or company with whom the School has contracted to perform a special task (such as an attorney, auditor, medical consultant, or therapist); or a parent or student serving on an official committee, such as a disciplinary or grievance committee, or assisting another school official in performing his or her tasks. A school official has a legitimate educational interest if the official needs to review an education record in order to fulfill his or her professional responsibility.

Other Schools: The School may disclose education records without consent to officials

of another school district in which a student seeks or intends to enroll.

4) The right to file a complaint with the U.S. Department of Education concerning alleged failures

Washington County School District PPRA NOTICE AND CONSENT/ OPT-OUT FOR SPECIFIC ACTIVITIES

The Protection of Pupil Rights Amendment (PPRA), 20 U.S.C. § 1232h, requires the Washington County School District to notify you and obtain consent or allow you to opt your child out of participating in certain school activities. These activities include a student survey, analysis, or evaluation that concerns one or more of the following eight areas (“protected information surveys”):

1. Political affiliations or beliefs of the student or student’s parent;

2. Mental or psychological problems of the student or student’s family;

3. Sex behavior or attitudes;

4. Illegal, anti-social, self-incriminating, or demeaning behavior;

5. Critical appraisals of others with whom respondents have close family relationships;

6. Legally recognized privileged relationships, such as with lawyers, doctors, or ministers;

7. Religious practices, affiliations, or beliefs of the student or parents; or

8. Income, other than as required by law to determine program eligibility.

This requirement also applies to the collection, disclosure or use of student information for marketing purposes (“marketing surveys”), and certain physical exams and screenings.

The Washington County School District will provide parents, within a reasonable period of time prior to the administration of the surveys and activities, a notification of the surveys and activities and be provided an opportunity to opt their child out, as well as an opportunity to review the surveys. (Please note that this notice and consent/opt- out transfers from parents to any student who is 18 years old or an emancipated minor under State law.

Lava Ridge Safe Routes to School

Students Biking or Walking from Santa Clara

In the fall and spring, we have close to 200 students who walk or bike to school. To keep these students safe, it is very important that everyone follows the safe route to school plan. All of those students who are walking or biking to Lava Ridge from Santa Clara need to use sidewalks and use the crossing lights and signs at Pioneer Parkway and Rachel Dr. As students pass in front of the fire station please use the path after the fire station to connect up with the nice asphalt bike path that goes around the parking lot of Gubler park. Please do not cross the entrance to Gubler Park as that is a busy entrance in the a.m. hours. They will then walk/ride across the bridge onto school property.

Students Biking or Walking from Ivins

If you have a student who is walking or biking along 800 South to Lava Ridge, please have them use the sidewalk that has been constructed for this purpose. Walk your bike across the bus parking lot entrance and exit and walk it up the sidewalk where the busses park on the North side of the school. You will then ride your bike behind the school over to the bike parking zone. Please be cautious!

Student Drop-off

Parents who are coming from Santa Clara will drop off their students in the Gubler Park Parking lot up by the splash pad. The student will then walk across the bridge to LRI. Parents who are coming from Ivins will use the pull up zone in front of the school. Please pull all the way forward before dropping your child off to help prevent back-up and congestion. Please do not drop off your student(s) in the north or south school parking lots. These are faculty parking only and there is a student bike parking zone in the south lot. Do not drop-off in these areas. Please follow this plan to help alleviate traffic congestion.
