Welcome to the Corla Health (“Corla Health”) website located at www.corlahealth.com (the “Site”). Please read these Terms of Service (the “Terms”) and our Privacy Policy carefully because they govern your use of our Site and our pharmacy and prescription delivery services accessible via our Site and our web accessible application (“App”). To make these Terms easier to read, the Site, our services, and App are collectively called the “Services.”

This service is provided by CH Medical CA PC on the Corla Health platform (together referred to as ‘Corla Health’).

Our Services

Corla Health provides prescription delivery services and referral to telehealth providers for prescriptions in certain geographic areas. Once you create an account and provide us with certain information, you’ll be able to order and schedule delivery of prescriptions using the Services and connect to a telehealth provider for menopause, perimenopause, GSM-related symptoms secondary to breast cancer treatment or treatment with testosterone, or any other GSM or menopause-related symptoms (if needed).

When you use our Services to request to receive a prescription, we either fulfill or process your prescription through an independent “partner pharmacy” that processes and fulfills your prescription.

By using Corla Health, you consent to the following:

Agreement to Terms

By using our Services, you agree to be bound by these Terms and by our Privacy Policy. If you do not agree to these Terms and our Privacy Policy, do not use the Services.

Changes to Terms or Services

We may modify the Terms at any time, at our sole discretion. If we do so, we’ll let you know either by posting the modified Terms on the Site or through other communications. It’s important that you review the Terms whenever we modify them because if you continue to use the Services after we have posted modified Terms on the Site, you are indicating to us that you agree to be bound by the modified Terms. If you do not agree to be bound by the modified Terms, then you may not use the Services anymore. Because our Services are evolving over time, we may change or discontinue all or any part of the Services, at any time and without notice, at our sole discretion.

Who May Use the Services

ELIGIBILITY

The Pharmacy and prescription delivery services are only available in certain geographic areas. You can check our Site to see if your area is currently being serviced.

You may use the Services only if you are 18 years or older and capable of forming a binding contract with Corla Health and are not barred from using the Services under applicable law. [If you are under the age of 18, you may request services from Corla Health. However, the provision of services for individuals under 18 may only be provided if permitted under applicable state law and in compliance with the Corla Health requirements.]

Registration and Your Information

Before you can start using the Services, you have to create an account (“Account”). It’s important that you provide us with accurate, complete, and up-to-date information for your Account and you agree to update such information, as needed, to keep it accurate, complete and up-to-date. If you don’t, we might have to suspend or terminate your Account. You agree that you will not disclose your Account password to anyone and you’ll notify us immediately of any unauthorized use of your Account. You’re responsible for all activities that occur under your Account, whether or not you know about them.

Passwords, Usernames, and Accounts

Your Account gives you access to the services and functionality that we may establish and maintain from time to time and in our sole discretion. You may never use another User’s account without permission. You are solely responsible for the activity that occurs on your account, and are responsible for taking all reasonable steps to ensure that no unauthorized person shall have access to your password or Account. It is your sole responsibility to (1) control the disclosure and use of your Account and password; (2) authorize, monitor, and control access to and use of your Account and password; (3) promptly change your password if you feel it has become compromised; and (4) promptly inform us of any need to deactivate your Account entirely. You grant us and all other persons involved in the operation of the Services the right to transmit, monitor, retrieve, store and use your information in connection with the operation of the Services. You represent and warrant to us that all information submitted to us through your Account is current, accurate and complete and Corla Health does not assume any responsibility or liability for information you submit, your or third-parties’ use or misuse of information transmitted or received using the Services. Fees

You are solely responsible for the cost of your medication that you receive by using the Services. If you provide us with your insurance information, we or our partner pharmacy will bill the insurance company first for prescriptions. After confirming with you, we or our partner pharmacy will charge the cost of your prescription (including any copays) using the credit card you provide when you create an account. Faster delivery options or outside of service locations may incur additional charges. You will also be responsible for the fees related to any services you choose on the Site and any additional care you receive as indicated in the pricing section.

We reserve the right to change our fee structure at any time and will notify you and confirm your agreement to proceed before changes are charged to you.

Health-Related Content is not Professional Medical Advice

By using Corla Health’s Services, you agree that any health-related content found in the Services provides only general, reference information and is not intended to be specific guide for self-medication purposes or a substitute for professional medical advice. If you have questions, you should discuss the content provided with your primary care provider or another licensed healthcare professional. You should also check product information (including package inserts) regarding dosage, precautions, warnings, interactions, and contraindications before taking or using any device, medication, or supplement discussed on the Services.

You understand and agree that in no event will Corla Health be liable for any decision made or action taken in reliance on the information contained on, or accessible through, the Services. Reliance on any content provided by, or otherwise appearing on, the Services is solely at your own risk.

You understand and agree that neither Corla Health nor its partners, suppliers, or vendors are responsible for any claim, loss, or damage directly or indirectly resulting from your use of the Services or the information resources contained on or accessible through the Services.

Feedback

We welcome feedback, comments and suggestions for improvements to the Services (“Feedback”). You can submit Feedback by emailing us at team@corlahealth.com. You grant to us a non-exclusive, worldwide, perpetual, irrevocable, fully-paid, royalty-free, sublicensable and transferable license under any and all intellectual property rights that you own or control to use, copy, modify, create derivative works based upon and otherwise to exploit the Feedback for any purpose.

Privacy Policy

In order for us to provide the Services to you, you will need to provide certain personal information. The information you submit to us is governed by our Privacy Policy. Please refer to our Privacy Policy for information on how we collect, use and disclose information from you and our other users.

Content and Content Rights

For purposes of these Terms: (i) “Content” means text, graphics, images, music, software, audio, video, works of authorship of any kind, and information or other materials that are posted, generated, provided or otherwise made available through the Services; and (ii) “User Content” means any Content that Account holders (including you) provide to be made available through the Services. Content includes without limitation User Content. Reliance on the Content is solely at your own risk.

Content Ownership, Responsibility and Removal

Corla Health does not claim any ownership rights in any User Content and nothing in these Terms will be deemed to restrict any rights that you may have to use and exploit your User Content. Subject to the foregoing, Corla Health and its licensors exclusively own all right, title and interest in and to the Services and Content, including all associated intellectual property rights. You acknowledge that the Services and Content are protected by copyright, trademark, and other laws of the United States and foreign countries. You agree not to remove, alter or obscure any copyright, trademark, service mark or other proprietary rights notices incorporated in or accompanying the Services or Content.

Rights in User Content Granted by You

Subject to our obligations under the federal Health Insurance Portability and Accountability Act and other applicable laws, by making any User Content available through Services you hereby grant to Corla Health a non-exclusive, transferable, sublicensable, worldwide, royalty-free license to use, copy, modify, create derivative works based upon, distribute, publicly display, publicly perform and distribute your User Content in connection with operating and providing the Services and Content to you.

Rights in Content Granted by Corla Health

Subject to your compliance with these Terms, Corla Health grants you a limited, non-exclusive, non-transferable, non-sublicensable license to download, view, copy, display and print the Content solely in connection with your permitted use of the Services and solely for your personal and non-commercial purposes.

Prohibited Conduct

You agree that you will not initiate, engage in, or encourage any Prohibited Conduct in connection with your use of our Services. “Prohibited Conduct” includes doing things like:

Enforcement by Us

We have the right (but not the obligation) to review any or all portions of your Content and delete (or modify) any of your Content from our Services for any reason, including if we believe, in our sole judgment, your Content violates these Terms, or that we believe constitutes or promotes Prohibited Conduct or otherwise threatens the safety of, or harms any other person, or creates liability for us or any other person. We reserve the right (but have no obligation) to investigate and take appropriate action, including removing your Content from our Services (or modifying it), suspending or terminating your account and/or suspending or terminating the provision of our Services to you, and/or reporting you to law enforcement authorities, if you violate any provision of these Terms. In order to cooperate with governmental requests, subpoenas or court orders, to protect our systems, service providers, partners, and other users, or to ensure the integrity and operation of our business and systems, we may access and disclose any information or content we consider necessary or appropriate, including your account information (i.e. name, e-mail address, etc.), IP address and traffic information, usage history, your Content, and your conduct.

The Services and Site may contain links to third-party websites or resources. We provide these links only as a convenience and are not responsible for the content, products or services on or available from those websites, resources or links displayed on such websites, or security or privacy practices of third-party websites and do not make any representations regarding their content or accuracy. You acknowledge sole responsibility for and assume all risk arising from, your use of any third-party websites or resources. Links to third-party websites do not constitute or imply endorsement by us of those websites, the information they contain or any products or services they describe.

Termination

We may terminate your access to and use of the Services, at our sole discretion, at any time and without notice to you. You may cancel your Account at any time by sending an email to us at team@corlahealth.com. Upon any termination, discontinuation or cancellation of Services or your Account, all provisions of these Terms which by their nature should survive will survive, including, without limitation, Sections titled “Feedback,” “Content Ownership,” “Responsibility and Removal,” “Warranty Disclaimers,” “Limitation of Liability,” “General Terms.”

Warranty Disclaimers

THE SERVICES AND CONTENT ARE PROVIDED “AS IS,” WITHOUT WARRANTY OF ANY KIND. WITHOUT LIMITING THE FOREGOING, WE EXPLICITLY DISCLAIM ANY WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, QUIET ENJOYMENT OR NON-INFRINGEMENT, AND ANY WARRANTIES ARISING OUT OF COURSE OF DEALING OR USAGE OF TRADE. YOU ASSUME TOTAL RESPONSIBILITY AND RISK FOR YOUR USE OF THE SERVICES, ALL SITE-RELATED SERVICES, AND THIRD-PARTY WEBSITES. NO ORAL OR WRITTEN INFORMATION OR ADVICE GIVEN BY THE SERVICES OR ITS AUTHORIZED REPRESENTATIVES SHALL CREATE A WARRANTY OF ANY KIND. We make no warranty that the Services will meet your requirements or be available on an uninterrupted, secure, or error-free basis. Corla Health EXPRESSLY DISCLAIMS ANY REPRESENTATION OR WARRANTY, WHETHER EXPRESSED OR IMPLIED, REGARDING A PHARMACY’S PROFESSIONAL QUALIFICATIONS, EXPERTISE, QUALITY OF WORK, OR ACCURACY IN FULFILLING YOUR PRESCRIPTION. Corla Health EXPRESSLY DISCLAIMS ANY LIABILITY FOR ANY INCORRECTLY PROCESSED PRESCRIPTION BY THE RELEVANT PHARMACY OR ANY INACCURATE INFORMATION PROVIDED BY YOU. Corla Health EXPRESSLY DISCLAIMS ANY LIABILITY FOR CANCELLED OR OTHERWISE UNFULFILLED ORDERS OR ANY INJURY OR DEATH RESULTING THEREFROM.

WHEN YOU RECEIVE YOUR PRESCRIPTION, IT IS SOLELY YOUR RESPONSIBILITY TO ENSURE IT WAS PROPERLY FULFILLED AND THAT YOU CONSUME THE PROPER DOSAGE. ADDITIONALLY, IT IS YOUR SOLE RESPONSIBILITY TO STORE ALL PRESCRIPTIONS PROPERLY AND IN ACCORDANCE WITH THE GUIDELINES PROVIDED BY THE PRESCRIPTION MANUFACTURER. IF YOU HAVE ANY QUESTIONS REGARDING YOUR PRESCRIPTION, PLEASE CONSULT YOUR PHYSICIAN.

Limitation of Liability

NEITHER Corla Health NOR ANY OTHER PARTY INVOLVED IN CREATING, PRODUCING, OR DELIVERING THE SERVICES OR CONTENT WILL BE LIABLE FOR ANY INCIDENTAL, SPECIAL, EXEMPLARY OR CONSEQUENTIAL DAMAGES, INCLUDING LOST PROFITS, LOSS OF DATA OR GOODWILL, SERVICE INTERRUPTION, COMPUTER DAMAGE OR SYSTEM FAILURE OR THE COST OF SUBSTITUTE SERVICES ARISING OUT OF OR IN CONNECTION WITH THESE TERMS OR FROM THE USE OF OR INABILITY TO USE THE SERVICES OR CONTENT, WHETHER BASED ON WARRANTY, CONTRACT, TORT (INCLUDING NEGLIGENCE), PRODUCT LIABILITY OR ANY OTHER LEGAL THEORY, AND WHETHER OR NOT Corla Health HAS BEEN INFORMED OF THE POSSIBILITY OF SUCH DAMAGE, EVEN IF A LIMITED REMEDY SET FORTH HEREIN IS FOUND TO HAVE FAILED OF ITS ESSENTIAL PURPOSE. SOME JURISDICTIONS DO NOT ALLOW THE EXCLUSION OR LIMITATION OF LIABILITY FOR CONSEQUENTIAL OR INCIDENTAL DAMAGES, SO THE ABOVE LIMITATION MAY NOT APPLY TO YOU.

IN NO EVENT WILL CLIENT’S TOTAL LIABILITY ARISING OUT OF OR IN CONNECTION WITH THESE TERMS OR FROM THE USE OF OR INABILITY TO USE THE SERVICES OR CONTENT EXCEED THE AMOUNTS YOU HAVE PAID TO Corla Health FOR USE OF THE SERVICES OR CONTENT OR ONE HUNDRED DOLLARS ($100), IF YOU HAVE NOT HAD ANY PAYMENT OBLIGATIONS TO Corla Health, AS APPLICABLE.

THE EXCLUSIONS AND LIMITATIONS OF DAMAGES SET FORTH ABOVE ARE FUNDAMENTAL ELEMENTS OF THE BASIS OF THE BARGAIN BETWEEN Corla Health AND YOU.

General Terms

Governing Law

These Terms will be governed by and construed in accordance with the laws of the State of California, without regard to or application of conflicts of law rules or principles. By using our Services you consent to personal jurisdiction and venue in the state and federal courts for San Francisco County, California for any lawsuit filed there against you by us arising from or relating to these Terms or our Services. You agree that San Francisco County, California is the proper forum for any appeals of an arbitration award or for trial court proceedings in the event that the arbitration provision below is found to be unenforceable.

Class Action/Jury Trial Waiver

With respect to all persons and entities, regardless of whether they have obtained or used the service for personal, commercial or other purposes, all claims must be brought in the parties’ individual capacity, and not as a plaintiff or class member in any purported class action, collective action, private attorney general action or other representative proceeding. This waiver applies to class arbitration, and, unless we agree otherwise, the arbitrator may not consolidate more than one person’s claims. you agree that, by entering into this agreement, you and Corla Health are each waiving the right to a trial by jury or to participate in a class action, collective action, private attorney general action, or other representative proceeding of any kind.

Entire Agreement/Severability

These Terms constitute the entire and exclusive understanding and agreement between Corla Health and you regarding the Services and Content, and these Terms supersede and replace any and all prior oral or written understandings or agreements between Corla Health and you regarding the Services and Content. If for any reason a court of competent jurisdiction finds any provision of these Terms invalid or unenforceable, that provision will be enforced to the maximum extent permissible and the other provisions of these Terms will remain in full force and effect.

Assignment

You may not assign or transfer these Terms, by operation of law or otherwise, without Corla Health’s prior written consent. Any attempt by you to assign or transfer these Terms, without such consent, will be null. Corla Health may freely assign or transfer these Terms without restriction. Subject to the foregoing, these Terms will bind and inure to the benefit of the parties, their successors and permitted assigns.

Notifications

Any notices or other communications provided by Corla Health under these Terms, including those regarding modifications to these Terms, will be given: (i) via email; or (ii) by posting to the Site. For notices made by e-mail, the date of receipt will be deemed the date on which such notice is transmitted.

No Waiver

Corla Health’s failure to enforce any right or provision of these Terms will not be considered a waiver of such right or provision. The waiver of any such right or provision will be effective only if in writing and signed by a duly authorized representative of Corla Health. Except as expressly set forth in these Terms, the exercise by either party of any of its remedies under these Terms will be without prejudice to its other remedies under these Terms or otherwise.

Discount Code Terms & Conditions

Discount codes are valid for a limited time only. Corla Health reserves the right to modify or cancel coupons at any time.

Information You Provide

We will collect personal information from you (like your name, date of birth, email address, credit card information, phone number, and delivery address) when you register for an account, and, in order to provide you with the prescription delivery services, you will need to provide us with certain medical information such as your clinician’s name and address, prescription information, insurance information, and in some cases, medical history (collectively, your “Personal Information”).

Note regarding children: We do not knowingly collect Personal Information from children. If we learn that we have collected Personal Information of a child under 13, we will take steps to delete such information from our files as soon as possible.

Note regarding International users: Your information will be transferred to and maintained on computers located in the United States (or any other country where we operate) and processed there – by providing us with your information, you are accepting and agreeing to that transfer.

Information We Collect

We collect certain information that your mobile device sends when you use our Services, like a device identifier, user settings, as well as information about your use of our Services.

We also collect certain information that your web browser sends when you visit our website, like the Internet Protocol (IP) address, browser, referring/exit pages and URLs, pages viewed, and other such information about your use of our Services. We use “cookies” – small data files – to collect this information, which allows us to identify your browser and to improve your use of our Services. You can reset your web browser to refuse all cookies or to indicate when a cookie is being sent, however, some features of our Services may not function properly if the ability to accept cookies is disabled.

We use the information we collect to provide our Services (or the information you request), including fulfilling your prescriptions and delivering them to you, to process and complete any transactions, to respond to inquiries, to personalize and improve our Services and your experiences when you use our Services, to monitor and analyze usage and trends of our Services, to send you administrative messages regarding the operation and use of our Services, and for any other purpose for which the information was collected.

Information We Share With Others

In order to fulfill and deliver your prescription, we may share your Personal Information with a third party pharmacy (“Pharmacy”). The Pharmacy will then use your Personal Information to fulfill your prescription. From the Pharmacy, we may receive your address, physician’s name, prescription information, prescription refill orders, and information on how to hand your prescription.

Your information may be accessed and used by our service providers who are working with us in connection with the operation of our Services (these service providers may have access to your information but only to the extent necessary to perform services on our behalf and are obligated not to disclose that information or use it for any other purposes).

We may share information about you if we are (or if we believe we are) required by law or legal process (such as a subpoena, warrant or court order), if we have to respond to a lawful request from legal authorities to disclose such information, or if we need to enforce or apply this Privacy Policy, our Terms or our other policies.

We may transfer and/or provide information about our users in connection with an acquisition, sale of company assets, or other situation where user information would be transferred as one of our business assets.

Our Site does not have the capability to respond to “Do Not Track” signals received from various web browsers.

How to Access Your Information

You can access and modify the information you provided us through your account. If you completely delete all such information, then your account will become deactivated and you will not be able to access our Services. If you would like us to delete Personal Information about you from our system, please contact us with a request that we delete your Personal Information from our database. We will use reasonable efforts to honor your request; however, we may retain an archived copy of your information as required by law and/or for record keeping purposes.

Your California Privacy Rights

California Civil code Section 1798.83, entitles California customers to request information relating to whether a business has disclosed personal information to any third parties for the third parties’ direct marketing purposes. As explained elsewhere on this Site, Corla Health will not sell or transfer your personal information to third party companies for their direct marketing purposes without your consent. Security Measures We Take To Protect Your Information

We employ administrative, physical and electronic measures designed to protect your information from unauthorized access, however, despite these efforts, no security measures are perfect or impenetrable and no method of data transmission can be guaranteed against any interception or other type of misuse. In the event that your Personal Information is compromised as a result of a breach of security, we will promptly notify you if your Personal Information has been compromised, as required by applicable law.

NOT FOR EMERGENCIES

I understand that I should never use Corla Health in a medical or psychiatric emergency. I understand that in an emergency, I should dial 911 or go to an emergency department.

WHEN OUR DUTY OF CARE BEGINS

I understand that Corla Health licensed medical providers (‘the Provider’) will take responsibility for my care only after I have created an account, answered all the required health questions, and made payment, and the Provider has subsequently reviewed my request for treatment and the health questions that I have completed, reviewed all my information, and then subsequently determined that I am a good candidate for the telehealth services. I understand that the duty of care does not begin at the point of me answering questions or making payment but at the point at which the Provider accepts the duty of care.

I understand that the Provider has the right to refuse to take responsibility for my care if the Provider makes a professional judgment that I am not a good candidate for this service. I understand that making a request for treatment (by completing a visit in the Corla Health Website (the “Website”) and making payment) or sending a message through the website/app does not in and of itself create a duty of care nor create a Provider-patient relationship.

I understand that there may be a delay 1-2 business days before a Provider reviews my request for treatment and any messages I send.

I understand that the only content in the Website that constitutes professional medical advice is the personalized messages the Provider sends me (once I have completed the health questions and made payment, and the Provider has subsequently taken responsibility for my care) and any content that the Provider links to in such messages. No other content in the Website constitutes professional medical advice. Specifically, the information provided in our health questions about who we can and cannot treat does not constitute professional medical advice.

I understand that all other content on the Website does not constitute professional medical advice and is instead for information purposes only. Never disregard professional medical advice or delay in seeking it because of something you have read on our Website.

BENEFITS AND RISKS OF USING OUR SERVICE

I understand that by using the service I’m seeking quality care that’s convenient and affordable.

I understand that important differences exist between Corla’s model of care and traditional healthcare. Specifically by using Corla, I accept a far greater responsibility to read and understand information throughout the Website about the limitations of Corla’s model of care, the risks of seeking care this way, and the risks and benefits of a proposed treatment plan.

I understand that I must read and understand the Consent to Telehealth, the Terms of Use, the Privacy Policy, the information provided about a service before I answer health questions, the information provided in the health questions themselves, messages I receive from Corla, and, very importantly, content provided via links in the message that the Provider sends me after the Provider has reviewed all my information and recommended a treatment plan and, when appropriate, prescribed a medicine.

I understand that to read important information I may need to both click on links and various titles to expand the information that’s visible below, and that without clicking on links and titles I will not be able to read important information that enables me to give my informed consent to a treatment.

I understand that by using Corla I accept the responsibility to provide full and truthful answers to all questions and, when requested, to provide unaltered photos of me that are taken at the time of using our service.

I understand that the Provider is unable independently to verify the information and photos I provide and that the Provider will make a professional judgment based on the information and photos I provide.

I understand that I won’t receive any other medical services that go beyond the diagnosis and prescription for GSM or menopause- or GSM-related symptoms. I need to seek other sources of care for my other medical needs.

I understand that by using the service for a telemedical consultation, I will NOT have an in person consultation nor a physical exam that might identify a medical condition that needs further investigation or immediate treatment.

I understand that by using the service I won’t speak or message with a Provider or nurse in real time.

I understand that I must check the Website for messages because this is the way that the Provider will communicate important information to me. I understand that if I don’t check the Website regularly, then my care may be delayed.

I understand that if I have any questions relating to my care that are not urgent, I can message the Provider through the Website. I understand that a Provider may not review and respond to my messages until the next business day.

IMPORTANCE OF READING ALL THE INFORMATION WE PROVIDE

I understand that Corla will provide detailed information on the Website to help me make an informed decision about whether to accept a proposed treatment plan. The most important information about a treatment plan is in the link that the Provider will send me when the Provider prescribes a treatment. This information includes detailed information to help me decide if the benefits of the treatment plan outweigh the risks, given the alternative options available to me, which includes the option of not taking any treatment.

I understand the importance of reading the information the Provider provides about adverse events, including the signs and symptoms of serious side effects and common side effects from taking a medicine, as this will ensure that I seek appropriate medical attention in a timely manner.

IMPORTANCE OF ANSWERING ALL QUESTIONS FULLY AND TRUTHFULLY

I understand that by using Corla I seek to enter into a relationship where the Provider relies exclusively upon information and photos that I provide to decide whether or not treatment is safe and appropriate.

I understand that the Provider has no way of verifying the information and photos that I provide and that the Provider will consider information to be accurate, true and complete, including my age, gender and all my answers to health questions, and the photos to be of me, taken at the time of me using the service, and unaltered.

I understand that if I provide information that is not true or incomplete, then I’ll be at greater risk of adverse events from any treatment that the Provider prescribes and I may receive a treatment that is unnecessary, inappropriate, or unsafe.

I understand that if I provide photos that are altered, not of me or not taken at the time of me using the service, then I’ll be at greater risk of adverse events from any treatment that the Provider prescribes and I may take a treatment that is unnecessary, inappropriate, or unsafe.

I understand that even if I provide information that is true and complete, I’m still at risk of adverse events from any treatment that the Provider prescribes.

I understand that even if I provide photos that are unaltered, of me and taken at the time of using the service, I’m still at risk of adverse events from my treatment that the Provider prescribes.

I understand that it is important that I do not create more than one account. Creating more than one account makes it impossible for the Provider to see the full history of care that I have received from Corla. This increases the chances that the Provider will not have access to important information and photos in my medical record that could influence the Provider’s clinical decision.

RISKS OF ACCEPTING OUR TREATMENT PLAN

I understand that all the medicines that the Provider may prescribe can cause serious side effects and adverse events that include severe allergic reaction, permanent disability, and death.

I understand that it is my responsibility to make an informed decision whether to accept a treatment plan that the Provider proposes after weighing the risks and benefits of the medicine being prescribed, alternative treatment options and the risks and benefits of such alternatives, and the option of not seeking any treatment.

I understand the importance of reading the manufacturer’s leaflet that comes with a medicine before I take a medicine because this leaflet includes important information about risks and warnings.

I understand that adverse events can be caused by a number of things, including an allergic reaction, side effects, undiagnosed blood disorder/medical condition, or interactions between a medicine that the Provider prescribes and any medical conditions I may have, other prescription medicines or other things (e.g., supplements, herbs, over-the-counter medicines, or recreational drugs) I’m taking, and lifestyle choices such as smoking tobacco products or drinking alcohol.

I understand that if I take systemic hormonal treatment without having had a mammogram according to current guidelines,  then I have an increased risk of breast cancer.

RISKS TO ELECTRONIC HEALTH INFORMATION AND DATA SECURITY

We have implemented a wide range of administrative, physical, and technical safeguards to reasonably secure your Personal Health Information (PHI) from accidental loss and from unauthorized access, use, alteration, and disclosure and to comply with HIPAA.

You have chosen a password which enables you to access certain parts of our Platform and you are responsible for keeping this password confidential. Do not share your password with anyone. The information you share in public areas (such as comments on blogs, videos, etc.) may be viewed by any user of the Platform.

When you use our Platform there is a risk that your PHI will be stored unencrypted on your mobile device. Unfortunately, the transmission of information via the Internet is not completely secure. Although we will do our best to protect your Personal Health Information, we cannot guarantee the security of your data transmitted to our Platform; any transmission is at your own risk. We use a variety of technical safeguards to maintain the security of your information and to prevent unauthorized access to it by those who do not have a legitimate need for it. In addition, we make sure to comply with HIPAA law and we periodically review our security measures. We will never post your e-mail address or other identifying information you have provided on our App without your consent. Please note that no method of transmission over the Internet or method of electronic storage is 100% secure.

Corla Health’s Menopause Membership services include asynchronous online Provider consultations, communication services, and other technology-enabled or personal support.

If you’re unhappy with the care plan offered after your initial assessment, let us know at team@corlahealth.com within 1 week of your initial assessment to get your money back. Cost of medication not included.

Pausing Your Membership

If you need to pause your membership-only plan, please contact our support team at team@corlahealth.com and specify the date until which you’d like to pause.

Your membership will remain active during the pause period, but medication refills will not be sent.
