Entering Hearing and Vision Screening Results
Synergy, SFUSD’s Student Information System, is designed to hold needed health information for each enrolled student. Below are instructions on how to enter hearing and vision screening information
Entering Vision Screening Results
The Vision tab records the results of a student’s vision exam, including if a student is referred to
Ophthalmologist.
- Navigate to Synergy SIS > Health > Health Screen.
- Select the Vision tab.
- Click Add Wizard on the Vision grid. The Health Screen Vision Detail Add screen displays.

- Add the appropriate information about the vision test.

- Screen Date – the date the test was administered
- Grade – the grade the student was in when the test was administered
- Staff Name – the staff member who administered the test
- Vision Aid – indicate if the student wears contacts or glasses
- Color Deficiency – indicate if the student has passed the color blindness test
- Ocular Alignment – indicates the results of the cover test
- Referral – indicate if the student was referred to an ophthalmologist
- Referral Date – the date the student was referred
- Referral Results – indicate results of the referred examination (after the screening letter is returned)
- Reason – indicate if the student was referred by the health staff, teacher, or Parent (optional)
- Near Left Eye, Near Right Eye, Near Both Eyes – indicate the student’s results
- Without Glasses – the vision test results without the student wearing corrective lenses. Select either PASS or FAIL. Do not enter the exact acuity.
- With Glasses – the vision test results with the student wearing their current corrective lens prescription. Select either PASS or FAIL. Do not enter the exact acuity.
- Comments – record any comments about the eye examination, including specific acuity if you wish
- Click Save.
Note: You can also enter vision-screening results using by clicking Add on the Vision grid. It adds a row to the Vision grid, where you add the basic information about the exam. Click Show Detail to add details.
Entering Hearing Screening Results
The Hearing tab records the results of a student’s audio exam, including if a student is referred
to an audiologist.
- Navigate to Synergy SIS > Health > Health Screen.
- Select the Hearing tab.
- Select the appropriate options regarding the student’s current auditory condition.

- Medical Documentation Received – indicates if any medical documentation indicating that the student has an auditory issue has been received
- Re-Evaluation Letter – indicates if the parents have been sent a letter indicating that the student needs to be re-evaluated
- Parent Refuses Clinic – indicates if the parents have refused to have their student participate in the auditory screening
- Evaluated by Audiologist – indicates if the student was evaluated by an audiologist
- Evaluated by Medical Provider – indicates if the student was evaluated by another medical professional (enter if the screening letter is returned)
- Preferential Seating – indicates if the student should receive preferential seating because of their auditory issue
- Ok for Educational Evaluation – indicates if the student’s parents have given permission for an educational evaluation
- Hard of Hearing Date – the date the student was determined to be hard of hearing
- Comment – general comments regarding the student’s auditory issues
- Click Add Wizard on the Audio grid. The Health Screen Audio Detail Add screen displays.
- Add the appropriate information about the hearing test.

- Screen Date – the date the test was administered
- Clinic Date – the date the auditory clinic was held
- Grade – the grade the student was in when the test was administered
- Staff Name – the staff member who administered the test
- Referral Date – the date the student was referred
- Referral – indicate if the student was referred to an audiologist
- Reason – indicate if the student was referred by the health staff, teacher, or parent
- Left Ear, Right Ear – indicate the results of various auditory measurements; select PASS or FAIL
- Doctor Comments – record the doctor’s comments about the auditory examination
- Screener/Audiologist Comments – record the screener’s or audiologist’s comments about the auditory examination
- Click Save.
Note: You can also enter hearing screening results by clicking Add on the Audio grid. It adds a row to the Audio grid, where you add the basic information about the exam. Click Show Detail to add details.
Adding Health Screenings By Section
Usually you administer a health screenings to an entire class, usually during a health class or
physical education. The Health Screen By Section screen enables you to search by section,
and administer the test to all the students in that section. You can also filter some tabs by date,
so you can see the results (by class) of a screening administered on a specific date.
Administering Vision Screening By Section
- Navigate to Synergy SIS > Health > Health Screen By Section.

- Scroll to or find the appropriate section.
- Select the Vision tab. The students in the section display in the Vision Exam grid.
- Enter the appropriate information in the fields.
- Without Glasses – the vision test results without the student wearing corrective lenses
- With Glasses – the vision test results with the student wearing their current corrective lens prescription
- Click Save.
Adding additional vision exam details
- Select a student from the Student Name column on the Vision Exam grid.
- Click Show Detail. The Vision Detail tab displays.
- Enter the appropriate information in the fields.
- Screen Date – the date the test was administered
- Grade – the grade the student was in when the test was administered
- Staff Name – the staff member who administered the test

Tip: You can use the filter to display screening information for a specific date. You can also show students who do not have any screening information recorded for a specific date.
Tip: You can also access the additional fields by clicking on the student’s name in the Student Name column. The system displays the Health Screen for the selected student.

- Navigate to Synergy SIS > Health > Health Screen By Section.
- Scroll to or find the appropriate section.
- Select the Hearing tab. The students in the section display in the Audio Exam grid.
- Enter the appropriate information in the fields.
- Reason – indicate if the student was referred by the health staff, teacher, or parent
- Left Result – indicate the results of various auditory measurements; select PASS or FAIL
- Right Result – indicate the results of various auditory measurements; select PASS or FAIL
- Clinic Date – the date the auditory clinic was held
- Referral – indicate if the student was referred to an ophthalmologist
- Referral Date – the date the student was referred
- Click Save.

- Click Show Detail. The Audio Detail tab displays.
- Enter the appropriate information in the fields.
- Screen Date – the date the test was administered
- Clinic Date – the date the auditory clinic was held
- Grade – the grade the student was in when the test was administered
- Staff Name – the staff member who administered the test
- Referral Date – the date the student was referred
- Referral – indicate if the student was referred to an audiologist
- Reason – indicate if the student was referred by the health staff, teacher, or parent
- Folder - indicates to which folder these results are assigned
- Doctor Letter – indicates if any medical documentation indicating that the student has an auditory issue has been received
- Re-Evaluation Letter – indicates if the parents have been sent a letter indicating that the student needs to be re-evaluated
- Parent Refuses Clinic – indicates if the parents have refused to have their student participate in the auditory screening
- Left Ear, Right Ear – indicate the results of various auditory measurements; select PASS or FAIL
- Doctor Comments – record the doctor’s comments about the auditory examination
- Screener/Audiologist Comments – record the screener’s or audiologist’s comments about the auditory examination
- Click Save.
Tip: You can also access the additional fields by clicking on the student’s name in the Student Name column. The system displays the Health Screen for the selected student.
ACCESSING REPORTS
The Health module has various reports available. Individual reports print out information about a
single student per page, but you can print them for multiple students at one time. List reports
generate a list of all the students and their information as specified by the description of the list
report. Summary reports generate summaries for multiple students, without including student names..
- Navigating to Synergy SIS > Health > Reports.
- Click on the name of the report.
- Select the options to generate the report (listed in the following sections.)
- Click Print. The report prints as a PDF file to the screen, which you can send to the printer.
HLT409 – Vision Section List Report
The Vision Section List report prints a page for each class listing all of the students in the class,
with spaces to record their screening results. If the student participated in the vision screening,
their results will display on the report.
You can filter this report using the following settings on the Options tab:

- Section ID – filters the report by a single section
- Hide Students with Records for Screen Date - includes only the records for students that have not been screened on this date
- Screen Date - enter the date on which the screening was (or will be) conducted
HLT410 – Audio Section List Report
The Audio Section List report prints a page for each class listing all of the students in the class,
with spaces to record their screening results. If the students participated in the audio screening,
their results will display on the report.

You can filter this report using the following settings on the Options tab:

- Screen Date - enter the date on which the screening was (or will be) conducted
