FORMS:

  • Court order for adult name change (and gender marker change): Full packet of court forms for adult name change. The fee is $324. You can apply for a fee waiver (in forma pauperis) by reading the instructions and using the forms here. You can request the court order a change to your gender marker on the same forms/at the same hearing. The hearing date will probably be set within 6 weeks of your submission, and will be scheduled for another 1-2 months beyond that time
  • Court order for minor name change:  Full packet of court forms name change for a minor. Note, for a minor, you also need to publish notice
  • Gender marker on birth certificate: If you have a MN birth certificate, you can use the name change form to order an amendment to your birth certificate -- to both your name and gender marker. It appears, however, you do not NEED the court order to change your gender marker. You will need a letter from a doctor, however. More information here. 
  • Gender marker on state ID or driver’s license: You can also change your gender on your driver’s license by submitting a certified copy of the court order OR “Medical certification of appropriate clinical treatment for gender transition in the form of an original letter from a licensed physician (the document must have enough biographical data (e.g., name and date of birth) to clearly identify the person.”

North Carolina (NC):

  • Adult name changes forms are available (please specify which county).
  • I will have minor forms (under 16 and 16-17 years old) available soon.
  • If you live in Mecklenburg County, the Self Help center will give you free name change packets during the hours of 8:30AM-12PM Monday through Friday.
  • North Carolina requires two background checks (state and federal) for anyone over the age of 16. SBI requires (1) fingerprint card and FBI requires (2) FBI cards.
  • The fee for a state background check (SBI) is $14 and take approximately 1-2 weeks.
  • The fee for a federal background check (FBI) is $18 and can take approximately 18 weeks. If you use a channeler (a FBI approved processing office) it can take 1-2 weeks. Cost varies between $40-$70.
  • If you choose to use the 18 week FBI option, send your form, fingerprint card, and fee in first, wait for your report to return, and then send out your SBI background check. Your SBI may expire otherwise, and you will have to reapply without a refund.
