What questions should I ask when calling my insurance company?

Do I have nutritional counseling coverage on my insurance plan?

If the insurance company asks for a CPT code please provide them with the codes 97802 & 97803. If they say you do not have coverage using those codes, NEXT ask them to check your coverage for the CPT codes: 99401, 99402, 99403 and 99404.

If the representative asks for a diagnosis code for preventative nutrition – please tell them the visit is coded with the ICD 10 code: Z71.3 or Z72.4. We always code your visit using preventative coding to maximize the number of nutrition visits you receive from your insurance carrier.

Notes:

Have I met my deductible?

In the event you have a deductible, we are unable to bill your insurance company directly for nutrition services. If this is your situation, you will be responsible for a payment of $260.00 at the initial visit and $120.00 for subsequent follow-up visits.

Do I need to meet my deductible before nutrition services are covered?

Select yes/no

We can provide you with appropriate documentation (receipt of nutrition counseling services) to help you “pay down” your deductible. You will be responsible for providing this information to your insurance company. Once you have met your deductible AND you have nutrition service coverage on your insurance policy, we can then bill your insurance directly.

Do I have a co-pay for nutritional counseling?

For most insurance companies, registered dietitian services are considered a specialist. Therefore, your specialist co-pay is applicable and is payable at the time of service. This information is often apparent on the front of your actual insurance card.
It is important to note, we will bill your insurance for preventative services and wait for the claim to be processed after your visit with us to determine if you are required to provide a co-pay. If you are required to pay a co-pay for nutrition services, we then charge your credit card you have on file with us.

Do I have a co-pay that is due during my appointment?

Select yes/no

How much is my co-pay?

$_______ per visit

Are telehealth (virtual appointments) covered by my insurance plan?

Due to the COVID-19 pandemic, insurance companies may cover nutrition services via telehealth. It is important to note that some insurance plans may have changed their policies surrounding virtual telehealth visits.
