PARENT’S CONSENT FORM

To the MariSi MAPEH Club,

       This is to inform you that I give my consent to the attendance and participation of my son/daughter                                             of                                    in the specified sport taking place on (Date) from specified timeframe of the chosen sport at the specified venue of sports (KES, KES Gym, Marikina Sports Center). 

Rest assured that I will exert my effort in reminding my child to observe the health and safety protocol prescribed by the Department of Health and the IATF.
