PARENT’S CONSENT FORM

To the MariSi MAPEH Club,

       This is to inform you that I give my consent to the attendance and participation of my son/daughter                                             of                                    in the Volleyball Tryouts taking place on July 29/30 from 4:00pm-6:00pm at the KES Gym

Rest assured that I will exert my effort in reminding my child to observe the health and safety protocol prescribed by the Department of Health and the IATF.

I will:
