Summer 2025 Checklist

FORMS TO BE COMPLETED THROUGH YOUR ONLINE YOHA ACCOUNT:

Complete by April 15th

▢        Submit 1st tuition payment

Complete by May 15th

▢        Submit 2nd tuition payment

Complete by June 1st

▢        Submit Camper Profile Form (a questionnaire to help us get to know your camper)

▢        Submit Dudley & Kiniya Canteen Form with deposit to the camper’s “Canteen Account”

▢        Submit Health Forms via link to CampDoc in your YOHA Account

 When completing the CampDoc Health Form you will need to:

• Input and Upload Insurance Information 
• Upload a copy of your campers health care provider generated yearly physical.
• Upload a copy of health care provider generated immunization record
• If applicable, submit Authorization for Administration of Medication Form (Note: this form must be signed by the Parent(s) AND Healthcare Provider)
• Fill out a general Health History for your camper
• Fill out a Diet and Allergies section
• Sign a Consent to Treat authorization

▢        Review & Submit Camper & Parent Code of Conduct Forms

▢        Review & Submit Dudley & Kiniya Family Guide

▢        Submit Travel Plans Form 

By Opening Day

▢        Practice your YOHA and get ready for an amazing summer!
