Climax-Shelly FFA/MN FFA Student Travel/Consent Form:

ND FFA requires that all members complete a travel/consent form. These must be filled out every year. If a student does not have this form completed, they will not be allowed to participate in any FFA events. The form this year is now DIGITAL. If you need any help with this, please see Mrs. Kieffer! The directions are as followed:

-Log into your AET Account at www.theaet.com

*Chapter Number: MN0059

Username: First Initial of your first name and Last Name (Ex: gkieffer)

Password: First initial of your first name and last name(Ex: gkieffer)

-Click on Profile

-Click Emergency Info and Permission Forms

*Fill out the information that you know.

*You must fill out the parent/guardian information so that your parent/guardian can get their email.

-Once completed, click on “Confirm and Go to Forms”

*Parents can then complete missing information and sign it. (They will get an email and they must confirm it for your form to be complete! Please check your spam email if the verification doesn’t show up within 5 minutes. The form is not complete until a parent/guardian verifies the information!)

If you are a new FFA member, you will need to notify Mrs. Kieffer that you need an account ASAP. She must create them for you!
