This picture symbolizes a guy who came back from war and lost all of his limbs. His other senses have disappeared when being blown up by a landmine. This guy can’t see, hear, feel, talk, but he can still think. He feels like an inbetween state of life and death and wants to just go to heaven. The guy only wants to die to go to a better place, meanwhile he is currently in a state of misery and distress. In the scene above, he is in the hospital repeatedly signalling them to “Kill me”.
