RankOne Checklist

MAKE SURE YOU HAVE ALL THINGS COMPLETED WHEN YOU SHOW UP FOR TRYOUTS ON

 AUGUST 3rd!!

  • Physical  - on official UIL form, both pages or sides filled out, all signatures completed (doctor and parent)

The rest are all submitted online on https://lewisvilleisd.rankonesport.com/New/NewInstructionsPage.aspx

  • Medical History Form - please just submit once, it then goes to the trainers for review, so it will say incomplete, but as long as you have submitted it, they have it. Submitting more than once does not help, it’s just more work for the trainers to sort through.

  • UIL Concussion
  • Acknowledgement of Rules
  • UIL Steroid Form
  • Extra Curricular Code of Conduct
  • Emergency Travel Card
  • Cardiac Awareness Form
  • Medical Release Info (HIPPA)

  3. Soft Tissue Work (Massage, Graston, Cupping, etc)
  4. Recovery (Normatec, GameReady)

  1. We have relationship with the following doctor offices and can typically get you in within 24-48 hours. This is the fastest and easiest way for a student to be seen by a physician in most cases.
  1. OrthoTexas – Dr Kevin Myers and Dr Keith Heier

  1. This is for concussions and general medical conditions (skin, rib injuries, etc)

Ultimately, Hebron athletic training is here to help you. Our goal is to help decrease student absences and family cost due to medical care (PT, doctor visits) and to ensure the safety and well-being of all Hebron student-athletes by providing quality health care on campus.
