Gaming Credit Redemption Form

Forest Terrace Heights Community League issues credits to gaming volunteers who volunteer at Forest Terrace Heights gaming events.

Credits can only be redeemed by approved AGLC not-for-profit groups with a gaming license.

Redemption of Credits

  1. Volunteer pays for the eligible expense & notifies organization that the purchase is being paid with gaming funds OR complete the Request for Reimbursement Form & request a cheque be sent directly to the organization with a gaming license.
  2. Volunteer acquires approval of reimbursement by the Bingo or Casino Director.
  3. Volunteer requests reimbursement by dropping off the original receipt & completed Request for Reimbursement to 10150 - 80 St (in the locked mailbox). See Request for Reimbursement below for information required.
  4. Volunteer receives payment via Canada Post in approximately 2-3 weeks from submission.
