Basketball Activity Waiver and Release of Liability

I, [                                               ], hereby acknowledge and agree to participate in basketball activities organized by Xzennis Brereton Training. In consideration for being allowed to participate in these activities, I hereby agree to the following:

I understand that basketball is a physically demanding sport and involves risks and dangers of serious injury, including but not limited to bodily injury, permanent disability, paralysis, and death. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by me as a result of participating in these activities.

I hereby waive, release, and discharge Xzennis Brereton Training, its officers, directors, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in basketball activities, whether caused by the negligence of the organization or otherwise.

I agree to indemnify and hold harmless Xzennis Brereton Training from any loss, liability, damage, or cost, including court costs and attorney's fees, that they may incur due to my participation in basketball activities, whether caused by negligence or otherwise.

In the event of an injury or illness, I authorize Xzennis Brereton Training and its representatives to obtain medical treatment for me, including transportation to a medical facility, as they deem necessary.

I have read this waiver and release of liability form, fully understand its terms, understand that I am giving up substantial rights by signing it, and sign it voluntarily and without any inducement.

This waiver and release shall be binding upon my heirs, next of kin, executors, administrators, and assigns.
