We’re looking for better integration of health care services and better use of technology across health and care.

There’s a real challenge to use technology better for those who use the service and for those benefit from the services that are provided.

One major driver to facilitate integration is the better care fund.

On the way to 2016 and the better care money there are 14 pioneer sites that drive forward with innovation.

Benefits of integrated care include a coordinated approach around users that brings together better skills leading to a better outcome for the individual.

There are 5 benefits: transparency, transactions, interoperability, patient control and reduce admin.

We are aware how easy some consumer transactions are in comparison to public sector services.

We know information sharing is one of the area’s we need to tackle to support integration.

NHS Organisations can submit 1 digital care record application.

Information is the connective tissue of integrated care

Funds for digital care are available for 2014/15 and 2015/16

A good proposal demonstrates strategic coherence, governance, leadership, change management, project management, sourcing, strategy and IT

ROI must be > 1.5% plus a benefits realisation plan

The joint proposal must be signed off by HWB’s.
