Overall Aim
- Create less confusion for all parties involved including hospitalists, ICU providers, CV surgery
providers, and the patient access center
- Streamline the transfer process to provide the best care for our patients
Summary
- Around 9 AM, H42-50 hospitalist pages APP HH1 for sign outs; APP might reach out as well
- APP HH1 places transfer order after the sign out
- If multiple patients, H42-50 hospitalist can take all or assign some to other H5000 rounders
- Please let patient access center know the room # and patient name as well as whether you are taking all of the transfers or assigning to other hospitalists
- APP might reach out to H42-50 hospitalist later in the day for late transfers
- After 4 PM, APP should go thru patient access center
- On weekends, there might not be APP HH1; in that case, reach out to CVICU attending for sign outs
- CV surgery is the primary team and hospitalist is the consultant
- CV surgery APPs attend the morning SW/RNCC rounds (10:30 AM)
