WE ONLY ACCEPTS ONLINE REGISTRATION
I- APPLICATION/REGISTRATION AND SUBMISSION OF REQUIREMENTS FOR THE
SACRAMENT OF CONFIRMATION
A. Recently issued 2 COPIES OF BAPTISMAL CERTIFICATES WITHIN 6 MONTHS
(annotated with..)
1. FOR CONFIRMATION PURPOSE (Original copy) AND
2. FOR MARRIAGE PURPOSE (Only applicable to Candidates who are getting
Married.) Photo copy only AND PRESENT THE ORIGINAL COPY DURING
YOUR KUMPIL SCHEDULE.
B. The applicant should already have his/her FIRST HOLY COMMUNION)
IF NOT, you may mention that during your confession and ask the priest what to do.
C. Fully accomplished Registration Form (which you can download from the FB
Page, Pinned on top).
For the Principal Sponsors:
**Two sponsors only (Male and Female/ both Female or Male)
Biological parents, siblings, in-laws, future wife/husband ARE NOT
ALLOWED TO BE YOUR SPONSORS.
Please choose someone whom you believe can guide and teach you about faith. You may choose from your uncles/aunties, family friends, and cousins; and MUST BE PRACTICING CATHOLIC faith AND ALREADY RECEIVE THE SACRAMENT OF CONFIRMATION. **Please take note that though we asked you to indicate your sponsors on the registration form, we would be requiring that only one sponsor will attend.
** ID is to be presented by the sponsor during the day of the confirmation.
D. Send Love offering via GCash (750php per candidate, please scan the G-cash QR
Code provided or GCash Account Number 09454153869 (HERBERT JOHN C.)
and upload the Payment Verification for proper acknowledgement and receipt.
F. For (12-21 years old) who are NOT RESIDING Philippines.
1.) PERMISSION LETTER FROM THE current PARISH PRIEST
2.) CERTIFICATE of ATTENDANCE on CATECHISM.
3.} BAPTISMAL CERTIFICATE WITH ANNOTATION FOR CONFIRMATION
II- Fill up and upload all the requirements via google forms.(Please use a desktop or laptop.)
(DOUBLE CHECK YOUR EMAIL ADDRESS, see to it that it is CORRECT)
III- AFTER SUBMISSION:
Please call NSSMA staff 09772409731 to check if your registration was received. Then wait for the slot and schedule-reply via email. This will be sent every Thursday LATE afternoon. Our reply contains your date/schedule, slot number and other details.
IV- Please DO NOT SUBMIT requirements in FB messenger/E-mail/Viber for these will not be processed. Only applications submitted via Google Forms will be accommodated.
V- Confirmation Candidate must be 12 years of age and above.
(A current School I.D. is to be presented for 12 to 21yrs old)
VI- Please come on time, late comers will not be accommodated.

3. BAPTISMAL Certificate with Purpose for MARRIAGE (Photo Copy) and PRESENT THE ORIGINAL COPY OF YOUR BAPTISMAL CERT. FOR MARRIAGE.
4. GCASH SCREENSHOT (Payment Confirmation)
5. VALID ID with Picture.
MISSING ANY OF THESE WOULD RESULTS TO INCONVENIENCE OR WILL NOT BE PROCESSED.

MISSING ANY OF THESE WOULD RESULTS TO INCONVENIENCE OR WILL NOT BE PROCESSED.
Kung may problema/pagkakaiba/pagkakamali sa inyong Baptismal Certificate, mabuting ipa-correct muna ito sa simbahang nagbinyag sa iyo. Ang NSSMA ay walang magagawang baguhin ano man ang nakatala sa iyong Baptismal Certificate. HINDI PO MAKUKUHA ANG CONFIRMATION CERTIFICATE KUNG HINDI KUMPLETO ANG MGA HINIHINGING REQUIREMENTS.
