1.  What do I want to accomplish/learn?  Why?

2.  Who else has accomplished the same goal?  Who will support me?  (Find a mentor.)

3.  What skills and resources do I need?  Create a list.

4.  What will I have to change? (Think about things in YOU that might need to change: i.e. behavior, habits, schedule, etc.)  

5.  What is my plan?  (You can brainstorm here.  When you are ready to get into a deeper plan, there is another worksheet for you to work on, too.)

6.  What small steps can I take?

7.  How have I been doing?  (Check in with yourself and your mentor.)
