  • Duly signed Grade 11 Grades (1st Sem & 2nd Sem)
  • Grade 12 Certificate of Enrolment signed by the school principal/Diploma (for graduates)
  • ALS PASSER
  • Certificate of rating signed by the principal
  • Certificate of eligibility for admission to college
  • NON-SENIOR HIGH SCHOOL GRADUATE (Old high school curriculum (1st year to 4th year)
  • Form 137 (Permanent Record)
  • TRANSFEREE
  • Transcript of Record

GENERAL REQUIREMENTS

Mandatory Requirements:

  • NSO/PSA Birth Certificate
  • 2x2 photo with white background and name tag (formal/decent attire)

Other Requirements (as applicable):

  • Persons with Disabilities (PWDs)
  • PWD ID, and;
  • Fit to Study Certification from a Physician/Psychologist/Psychiatrist

  • Dependents of SLSU Regular Employees (parents or spouse are employed at SLSU)
  • Birth Certificate/Marriage Certificate, and;
  • Certificate of Employment of the parent/s and/or spouse from SLSU HRMO
  • Alternative Learning Systems (ALS)passers
  • Certificate of Eligibility to Enroll College

  • Indigenous People (a person who continuously lived in an organized communities preserving their own unique culture, traditions and way of life (ex. Mamanwa, Mandaya, Igorot)
  • IP ID or Barangay Certificate

  • Winners of Sports, Culture and Arts (Regional and National)
  • Certificate of Awards
  • For team sports, certification of merit and assessment of SLSU trainers/coaches recommended by the Director, Sports Development and Cultural Affairs subject to the results of the official try-outs

  • Solo Parent / Raised by Solo Parent (a person raising a child without the help of a spouse/father)
  • Solo Parent ID issued by the DSWD

  • Victims of conflict and displacement (individuals who have experienced conflict, violence, or instability and were forced to leave their homes, resulting in temporary or long-term displacement)
  • Certification from DSWD as internally displaced person

  • Dependents of persons officially enrolled under the Enhanced Comprehensive Local Integration Program (E-CLIP) (A government initiative that provides assistance, protection, and reintegration support to former rebel surrenderees and their families as part of ongoing peace-building and community stabilization efforts)
  • Certification from DILG

  • Orphans (a person who is deprived of parental authority due to the death of both parents, and no surviving immediate relatives or next of kin have assumed responsibility)
  • Certification from DSWD

  • First Generation (first member of the immediate family, including parents, to enroll College)
  • Barangay Certificate subject to the verification of the Admission Committee.

DEFINITION OF TERMS

Senior High School- in reference to Republic Act No. 10533, also known as the Enhanced Basic Education Act of 2013, these are students who are in their Grade-12 of secondary education or have already graduated in Grade-12.

Non-senior High School - a graduate of the old high school curriculum (1st year to 4th year) whom are not part of the implementation of the K-12 system.

Transferee - previously enrolled in a different college or university and has completed some college-level courses.

Alternative Learning System (ALS) graduate - completed the Alternative Learning System (ALS) provided by the Department of Education

Persons with Disability (PWD) - in reference to Republic Act No. 7277, also known as the Magna Carta for Disabled Persons, is any person suffering from restriction or different abilities, as a result of a mental, physical, or sensory impairment, to perform an activity in the manner or within the range considered normal for a human being. Identified by the disability that falls under any of the following categories: visual, hearing, orthopedic, speech, learning, mental, and developmental disabilities. Moreover, the disability must be permanent, and it must be certified by a licensed physician or clinical psychologist.

Indigenous Person - individuals identified by their Indigenous Person ID issued by the National Commission on Indigenous Peoples (NCIP)

Dependents of SLSU Regular Employees - children of SLSU regular employees

National And Regional Winners Of Sports, Culture And Arts - awarded during national and regional events of sports, culture and arts such as - Eastern Visayas Regional Athletic Association (EVRAA), Pintados-Kasadyaan Festival, Sangyaw Festival, Visayas Islands Visual Arts Exhibition and Conference, National Music Competitions for Young Artists (NAMCYA) Regional Competition and others.

Solo Parent - identified under Republic Act No. 8972, also known as the Solo Parent Welfare Act of 2000. Must be recognized as a solo parent and secured a Solo Parent Identification Card (SPID) from the Department of Social Welfare and Development (DSWD) office in their respective city/municipality.

Orphans - a person who is deprived of parental authority due to the death of both parents, and no surviving immediate relatives or next of kin have assumed responsibility.

First Generation - first in their immediate family, including parents, to attend College.
