GOAL: To improve family and social association (social capital) in Allegan County.

PERFORMANCE MEASURES

By, December of 2017 Allegan County Health Rankings Health Indicator (Social Associations) will increase from 11.6 to 15.0.

By December of 2017 Allegan County Health Rankings Health Indicator (Single Parent Families) will decrease from 24% to 23%.

OBJECTIVE #1:

Increase access and utilization of resources for families.

Background: In discussion the Community Health Prioritization Steering Committee (CHPSC) identified that there were numerous resources available locally but they are under-utilized.  Socially isolated individuals have increased risk for poor health outcomes (House, JS. (2001). Social Isolations Kills, but how and why?. Psychosom Med. 63: 273. 274.)

OBJECTIVE #2:

Reduce Trauma Impact on Children and Families in Allegan County.

BACKGROUND: There was significant discussion around early appropriate evidenced based intervention in families with trauma impact. There is not a standard curriculum for early childhood providers from the different sectors to utilize.  Lack of consistent training and knowledge increases the impact of the trauma sustained.  This trauma leads to developmental and behavior issues in schools, mental health issues, and incarceration later in their life.

GOAL: To improve access to care within Allegan County.

PERFORMANCE MEASURES

By December 2017 Allegan County Health Rankings Indicator Access to Care (Dental) will decrease from 3,800persons per 1 dental provider to 3,000 persons per 1 dental provider.

By December 2017 Allegan County Health Rankings Indicator Access to Care (Mental) will decrease from 1,407 persons per 1 provider to 1,000 persons per 1 provider.

By December 2017 Allegan County Health Rankings Indicator Access to Care (Primary Care) will decrease from 3,614 persons per 1 provider to 3,000 persons per 1 provider.

By December 2017 Allegan County Health Rankings Indicator Access to Care (Uninsured) will decrease from 13% to 11%.

OBJECTIVE 1:  Increase awareness and utilization of preventative services to include primary care, dental, and mental health.

BACKGROUND: There was discussion that identified that many individuals within Allegan County do not know what preventative services are and the importance of them.  This could be related to lack of previous insurance, cultural, or transportation/access issue. Many individuals consume services during acute crisis.  This discourages providers from providing services in Allegan County.

OBJECTIVE 2: Work with existing organizations to strategically discuss opportunities to increase dental, mental, and primary care providers within the county.

BACKGROUND: The County Health Rankings data in Allegan County specific to persons to one provider is 3 times greater than Michigan or Best US Performer data. There is limited transportation infrastructure within Allegan County.  Allegan County is large and there are many densely populated areas on County borders. This is going to have to be a multi sector approach to adequately address this issue.

GOAL: Decrease obesity rate within Allegan County.

PERFORMANCE MEASURES

By December 2017 Allegan County Health Rankings Indicator Adult Obesity will decrease from 34% to 32%.

By December 2017 Allegan County Health Rankings Indicator Food Environmental Index  will maintain 8.6

By December 2017 Allegan County Health Rankings Indicator Physical Inactivity will decrease from 24% to 22%.

By December 2017 Allegan County Health Rankings Indicator Access to Exercise Opportunities) will increase from 73% to 75%.

OBJECTIVE 1:  Increase access and consumption of
healthier dietary choices.

BACKGROUND: There was discussion regarding what resources currently exist within Allegan County regarding access to healthier foods and physical activity.  Underutilization of current resources was identified; with recognition of the ability to provide to more individuals than currently serving.

BACKGROUND: Allegan County should be known for the excellent outdoor resources including parks, trails, water ways and bike trails to name a few; though they are underutilized .   Community oriented opportunities such as pools, field houses, tennis courts, and gymnasiums are available; including buildings to walk in during inclement weather but community members often are unaware.
