Lakeview Community Schools’ Student Enrollment Process

Welcome to Lakeview Community Schools!  We are committed to providing a quality academic program and many opportunities for co-curricular activities.  If you are a new student (or the parent of a new student), we invite you to review the following information:

Residents of Lakeview

Residents of Lakeview Community Schools may register students at any time.  A parent is required to be present when enrolling students.  Please provide the following documents:

1. Parent/Guardian will need to Proof of Identification
2.

4You will need your child’s original State Certified Birth Certificate.

Immunization Requirements: If your child is entering a Michigan school for the first time, please have your healthcare provider complete the immunization portion of the Health Appraisal form, or provide the school with a record showing that your child has received all of the required immunizations

Proof of immunizations, must be returned to the school your child will be attending on or before the first day your child attends school.

Below are additional forms that can be completed as they pertain to you and your child’s needs: 

2015-16 FREE AND REDUCED LUNCH APPLICATION (right click to open link)

Children need healthy meals to learn.  Lakeview Community Schools offers healthy meals every school day.  Elementary students preschool - 3rd grade may buy lunch for $2.00.  Students 4-12th grade may buy lunch for $2.30  Milk only is $0.45.   Your children may qualify for free meals or for reduced price meals.  Reduced lunches are $0.40.  If a doctor has determined that your child has a disability, and the disability would prevent the child from eating the regular school meal, the school will make any substitution prescribed by a licensed physician at no extra charge.  The physician's statement, including prescribed diet and/or substitution, must be submitted to the food service department at your school.  For further information, please call Carla Cooper (food service director) at (989) 352-8016.

iChat Form(right click to open link): Lakeview Community Schools requires that all volunteers will be checked through iChat, SOARS, and OTIS of Michigan for criminal background history, prior to volunteering for any student activities.  iChats are done through Lakeview High School office.  A copy of a valid driver’s license must be included with this form and can be copied in the school office. This check will cover all buildings in the district.  Please note: iChat/SOARS/OTIS checks are renewed on a yearly basis.

Transportation Handbook and Bus Registration (right click to open link): If your child will need bussing, this form will need to be completed for your child(ren).  

Consent to Administer Medication Form(right click to open link): If your child requires the administration of medication during the school day, this form must be completed on file in our school office.  

Asthma Emergency Action Plan(right click to open link): If your child needs an inhaler at school and/or utilizes an inhaler, this form must be completed and on file in our school office.

Allergic Reactions Care Plan(right click to open link): If your child has an allergy of any kind, this form must be completed.

Temporary Placement (30 day) for Special Education Services(right click to open link): If your child was receiving special education services in his/her previous school district, this form must be completed to continue services.  The school receives the Individualized Education Plan (IEP). Please note: This includes speech services.

Non - Resident of Lakeview (School of Choice)

Lakeview Community Schools will be participating in the School of Choice Program for the 15-16 school year, in as passed by the Michigan legislature and signed into law by the Governor.  A parent is required to be present when enrolling students.

To have your child considered for admission under this program, please complete the following form and return AS SOON AS POSSIBLE, but no later than Friday, September 11, 2015.
