Mobile Device Agreement and Technology Usage Form

Purpose

Ferris Independent School District (hereafter referred to as “Ferris ISD”) uses Instructional Technology as one way of enhancing the mission to prepare and inspire all students for lifelong success by teaching the skills, knowledge and behavior students will need as responsible citizens in the global community. Students learn collaboration, communication, creativity and critical thinking in a variety of ways throughout the school day. Excellence in education requires that technology is not only seamlessly integrated throughout the education program, but approached through 21st Century teaching methods. In an effort to increase access to those 21st century skills, Ferris ISD is promoting the individual use of technology as a way to empower students to maximize their full potential and prepare them for postsecondary and workforce careers.

An important component is educating students about their digital citizenship and appropriate online behavior. Ferris ISD will diligently review cyber-safety rules with students throughout the school year, and will offer reminders and reinforcement about safe, responsible, and appropriate online behavior.

All students and parents/guardians must adhere to the Mobile Device and Technology Usage Agreement Policies, Student Code of Conduct, Student Handbook, Acceptable Use Policy, Responsible Use Guidelines, Board policies, and federal/state laws. Failure to do so could result in disciplinary action against the student, revocation of privileges and technology use, applicable fines/fees and/or prosecution according to state or federal laws should it be warranted by the offense.

Device Specifics

The technology device(s) covered by this agreement is a mobile computing device. The mobile computing device along with charger, charging cord, and case are being issued to the student. It is the student's responsibility to care for the equipment on a daily basis and ensure that it is kept in a safe environment.

Any technical or mechanical issues with the mobile device must be reported to the school immediately or at the earliest time possible so that it can be repaired or replaced by the Ferris ISD Technology Department. Mobile devices may not be serviced or replaced at an Apple Store, Best Buy, or any variety of service shops. This equipment is, and at all times remains, the property of Ferris ISD. The mobile device is subject to inspection, damage assessment (fees may be applied), and can be confiscated at any time for vandalism, misuse, or failure to adhere to the Ferris ISD Acceptable Use Policy, The Responsible Use Guidelines, this form, or classroom rules.

The device and equipment will need to be returned in its original condition (minus normal wear and usage) if a student graduates, graduates early, withdraws, or is expelled from school prior to the end of the school year. As a general note, mobile devices turned in for repair or at the end of the year are serviced. The mobile device will be taken up at the end of each year and re-issued at the beginning of the following school year. The district will make its best effort to see that the same device is re-issued to the same student that used it in a previous year. 

Device Protection

Ferris ISD recognizes that with the ongoing use of a one-to-one technology initiative program there is a need to protect the investment in these mobile devices by both the District and the Student/Parent/Guardian. This document outlines and explains the purpose of  protecting these mobile devices against damage, vandalism, loss and theft, and describes the responsibilities of students and parents/guardians pertaining to this initiative.

Exempt Defects

Students will not be held responsible for manufacturer defects, worn out batteries, worn out keyboards (this does not include missing keys) and trackpads due to normal use, worn out styluses due to normal use, software or apps provided or installed by the district, or operating system defects, nor will their parents/guardians be held liable for such issues either.  These are considered exempt defect categories, and the costs of such issues will be covered by the manufacturer warranty or absorbed by the district.

Theft, Accidental Damage, or Loss

In the event of theft or loss, a police report  must be filed, and the case number must be given to the campus principal who will pass it on to the Technology Director. In the event of theft, loss, and accidental damage the district will cover 100% of the repair/replacement amount one time only, and a replacement device will be issued to the student. A $20.00 fee will be assessed for each subsequent theft, loss, or accidental damage incident.  If a stolen or lost device is recovered, it will be returned to the student and the replacement device taken back. If a fee was assessed, it will be refunded, provided the original device is in good condition; otherwise, damage fees will take effect.

Vandalism

If a student damages a device due to horseplay, wilfully damages or defaces a device that is assigned to him/her or one that is assigned to another student, there will be a $150.00 vandalism fee assessed against the offending student.

In an instance where a student vandalizes a device that is not their own, the same $150.00 vandalism fee will be assessed to the offending student.  Additionally, the offending student will not have access to technology until the vandalized device is repaired.  A replacement device will be issued to a student whose device was vandalized by someone else.

Disciplinary action according to the student code of conduct will be administered at Ferris ISD administration’s discretion. The Ferris ISD Police Department will be contacted and a criminal report will be filed in all instances of vandalism.

Additional Items

Students and parents/guardians will be responsible for the full cost for replacements of accessories at a cost of $30.00 for the charger and $20.00 for the case.

Additional Information

For instances of theft, loss, or other criminal acts, a police report must be filed by the student or parent/guardian with the Ferris ISD Police Department, and a case number must be provided to the district prior to device replacement.

The cost of repair for damage to a device may only be assessed by the Ferris ISD Technology Department Staff. Repairs may only be processed by Ferris ISD. Mobile devices may not be repaired outside of the Ferris ISD Technology Department.

Student Responsibilities

Your mobile device is an important learning tool and is for educational purposes only. In order to take your mobile device home each day, you must be willing to accept the following responsibilities:

The mobile device assigned to me is my responsibility, and I will not leave it in unsupervised areas.

I will bring the mobile device to school every day with a fully-charged battery.

I will treat the mobile device appropriately and will report any mechanical or technical issues to the school in a timely manner.

I will backup my data before turning in my mobile device for repair or at the end of the year.

I will ensure to the best of my ability that the district-owned mobile device is not damaged, lost, or stolen while it is issued to me.

I will not remove or modify the identification, inventory labels, or make changes to the home/lock screen that have been placed on the mobile device.

_________

I will not add stickers, labels, scratches, engravings, tags, or markings of any kind to the mobile device.

_________

I agree to use the mobile device only for appropriate, legitimate, and responsible communications.

_________

I will keep my accounts and passwords secure and will not share these with any other

student.

_________

I will not attempt to add, delete, access, or modify other user accounts on the mobile device if present.

_________

I will not modify the operating system or attempt to “jailbreak” (unlock and/or alter certain features of the operating system) the mobile device.

I will not attempt to change or delete the internal “name” of the mobile device.

_________

I will not attempt reset the mobile device back to factory settings.

_________

I will not attempt to remove or alter in any way any profiles on the mobile device preset by the district.

I will take no action that could interfere with the district’s network.

_________

I will return the mobile device, charger, charging cord, and case in good working order when requested or upon my graduation, expulsion, or withdrawal from school.

I must perform regular requested and mandatory updates of the mobile device operating system only when directed by the Technology Department.

I will keep my mobile device protected with a school-issued case at all times.

Parent/Guardian Responsibilities

Your student has been issued a mobile device, charger, charging cord, and case (if applicable) as part of this 1:1 program to improve and personalize his/her educational experience this year. It is essential that the following guidelines be followed to ensure the safe, efficient, and ethical operation of this device:

I will supervise my student's use of the mobile device at home. What my student does and where he/she goes online (on the Internet) at home is my responsibility as his/her guardian to supervise, not the responsibility of Ferris ISD.

I will ensure that my student reports any mechanical or technical issues to the school in a timely manner.

I will ensure that my student understands and abides by the Student Responsibilities outlined in this Technology Use Agreement and in the Ferris ISD Acceptable Use Policy and Responsible Use Document.

I agree that the mobile device, charger, charging cord, and case will be returned to the school when requested and upon my student's graduation, expulsion, or withdrawal from school. I further understand that if said device is not returned, I will be financially responsible for its replacement.
