We welcome your organization to Coburn United Methodist Church. In order that your planning proceeds smoothly, we ask that you complete this form so that we can be prepared for your arrival and so that you are aware of the guidelines associated with the use of space at Coburn.

_____Custodial Services is $25 per hour. Minimum of 2 hours
_____Open & Close beyond normal hours $25 per hour (Regular hours Mon – Fri 8:30 AM to 3 PM)

Comments/Concerns
By signature below and proceeding with the use of Coburn United Methodist Church Facilities, the Representative and the organization, group, or persons, agrees to the following Terms Of Use:
- Facility use will be in accordance with the Policy Statement for the Use of Physical Facilities of the Coburn United Methodist Church. (No alcohol use or smoking is permitted)
- Payment of scheduled fees will be required one week prior to use. Fees may be waived for members and/or at the discretion of the pastor or trustee.
- There is a $100 deposit required for the use of the facility. This will be returned if the building is clean when use is complete.
- All requests for use of any facilities of the Coburn United Methodist Church must be submitted in writing to the church office Requests inside 30 days are dependent upon staff availability.
- Requests will be granted on a first-come basis and will be granted only after review by the trustee representative on the Leadership Board or the Pastor.
- The organization, group, or persons agree to restore the facilities to the condition that existed prior to use.
- The use of Coburn United Methodist Church Facilities by the organization, group, or persons is at their own risk. The Coburn United Methodist Church will not be liable for any property loss or damage, or injury to any person(s) of the Organization or group using these facilities.
- The representative signing this form is responsible to inform all individuals of the organization or group using Coburn United Methodist Church Facilities of these Terms Of Use.
- The right to use church facilities may be revoked at any time.

BUILDING USE RULES
- All decorating and room arranging must be scheduled at least one week prior to the event and accomplished during regular business hours. Our facility is closed after 3 PM on Friday evenings.
- Arrangements for decorating or set ups the day before an event must be made through the office and are only permitted if our facilities are not in use and staff is available.
- A member is defined as an active participant in the organization or group requesting facility use or an immediate family member of a family seeking use.
- It is asked that equipment inside the building not be used outside and that the custodian care for the movement of chairs and tables between rooms.
- It is expected that all decorations, behavior and practices of individuals and groups using the facilities of the Coburn Campus be consistent with common United Methodist and Christian practices.
- The Coburn facility can place no more than 35 round tables in the gym which will seat 280 persons or 40 long tables which will seat 320 persons.
- Nothing may be hung from the ceiling or basketball backboards.
- It is expected that organizations or groups will only be in the areas reserved by them and not make use of any other areas of the church except when appropriate such as restroom use.
- Emergency exits may not be blocked anywhere in the building.
- Safe practices must be followed at all times.
- Video and Sound Equipment may not be used without a representative of the Coburn Church who will operate that equipment.
- No gambling, games of chance or raffles are permitted on the Coburn Campus by United Methodist Church Law.
- No smoking or vaping, use of alcohol or use of drugs is permitted on the Coburn Campus.
- Unless other arrangements are made, you are responsible for setup and cleanup of the area.
