The massage therapists practicing at Eagles Nest Massage Therapy, Inc. have a wide range of training and experience;  to learn about the level of training that your therapist has obtained please refer to our website at www.eaglesnestmassage.com  Every Eagles Nest Massage therapist is licensed by the city of Duluth which requires a minimum of 500 hours of training and at this time there is no licensing available by the State of Minnesota for massage therapists.  Therapists are also screened and mentored by Eagles Nest staff to insure that your session will be consistent with the professional quality Eagles Nest strives for.

THE STATE OF MINNESOTA HAS NOT ADOPTED ANY EDUCATIONAL AND TRAINING STANDARDS FOR UNLICENSED COMPLEMENTARY AND ALTERNATIVE HEALTH CARE PRACTITIONERS.  ANY STATEMENT OF CREDENTIALS IS FOR INFORMATION PURPOSES ONLY.

Under Minnesota law, a complementary and alternative health care provider who is not licensed by the state may not provide a medical diagnosis or recommend discontinuance of medically prescribed treatments.  If a client desires a diagnosis from a licensed physician, chiropractor, acupuncture practitioner, athletic trainer, nurse, osteopath, physical therapist, dietitian, nutritionist, or any other type of health care provider, the client may seek such services at any time.

  1. Complementary and alternative health care clients have the right to file a complaint with the practitioner’s supervisor, Marcy Kernez.  She can be reached at either address or phone number located at the top of this document.

  1.  Eagles Nest Massage Therapy, Inc. fees for services as of 9/1/2024 are as follows:

Wellness Session Cost

30 minute session  =  $60.00   60 minute session  =  $105.00   90 minute session  =  $145.00

45 minute session  =  $80.00   75 minute session  =  $125.00

All fees are subject to MN State, Duluth City, and St Louis Co. sales tax totaling 8.875%.  There are intro offers, membership and packages available.  Please ask your therapist or desk staff for further explanation in regard to these.  

The client will be given a 30 day notice of any changes of the above fee structure.

All fees are to be paid by cash, check or credit card (Visa or MasterCard).  Eagles Nest Massage Therapy, Inc. DOES NOT accept payment from any third party payers although will assist the client in any necessary documentation to provide the third party payer so that the client may be personally reimbursed for their payment of services.

  1. Eagles Nest Massage Therapy, Inc.’s goal is to provide the highest quality of therapeutic massage services, offering a variety of therapists and styles in a professional and relaxing environment.

  1. Clients have the right to complete and current information regarding the practitioner’s assessment and recommended service that is to be provided, as well as the expected duration of the service to be provided.

  1. Clients can expect to be treated with the utmost respect and courtesy while receiving treatment.  

  1. All records and transactions are to be confidential, unless the client has authorized release of information in writing, or otherwise provided by law.  Clients have the right to access their records in accordance with section 144.335 of the Minnesota statutes.

  1. The client has the right to choose freely among available practitioners at Eagles Nest Massage Therapy or others in the community and to change practitioners after services have begun.  The client has the right to coordinate transfer when there will be a change to another facility.  A release of information will be required for the transfer to be complete.

  1. Clients may refuse services or treatment, unless otherwise provided by law.

  1.  Clients have the right to assert their rights without retaliation.

Prior to the provision of any service, a complementary and alternative health care client must sign a written statement attesting that the client has received the complementary and alternative health care bill of rights.  Your signature on your initial health information indicates you have received and fully read this information.  You are welcome to take this information with you.
