Medical Documentation Requirements 

Physical Examination: 

The Massachusetts Department of Public Health Regulation 105 CMR 200 requires a physical examination for all new students during the following increments: 

● One year prior or 30 days after entry to the Wayland Public Schools

● Entering grades preschool, K, 4, 7, and 10

● Annually prior to participation in school sponsored athletics at the Middle and High School level, a student participating in interscholastic athletics must submit a current physical examination. According to MIAA Guidelines to qualify for each sport season, the physical must be dated within 13 months of the first day of practice. If a student's physical expires during the current sport season, a new physical needs to be submitted.

Required Immunizations: 

Massachusetts Department of Public Health Regulation 105 CMR 220 requires students to be immunized before starting school. An immunization certificate/record that includes the month, day and year the immunizations were administered needs to be submitted to and reviewed by the school nurse before the student begins school. 

Exemption - If there are medical reasons why your child has not been immunized, a certificate must be obtained from your physician and forwarded to the school nurse; this letter must be provided each year. For religious exemption, a letter must be provided annually, that states that the immunization(s) are against one's sincere religious beliefs. Please note that if there is an outbreak of a communicable disease, students with exemptions may be excluded for the duration of the incubation of that particular illness. 

For incoming kindergarten and preschool students: 

Please upload a copy of a recent physical exam from a pediatrician or primary care provider (within the last year) including immunization information, as well as complete and upload the student health history form located under "Forms" on the portal. 

Kindergarten entry also requires:

● lead screening, including the date and results of the blood test

● tuberculosis risk assessment

● vision screening

Please upload documentation of these screenings with the physical exam. 

For new students to the District: 

Please upload a recent physical exam from a pediatrician or primary care provider (within the last year) including immunization information, as well as complete and upload the student health history form located under "Forms" on the portal. 

For students entering grades 4, 7, 10 or those participating in school sponsored athletics: 

If you have not already done so, please upload a recent physical (within the last year) including immunization information. MIAA requires a physical exam within the last 13 months.

For students that require an EpiPen: 

We ask that you review the EpiPen program information document found under "forms" on the portal. Please complete and upload the following which may be found under forms: 

● EpiPen parent/guardian permission

● Allergy and Anaphylaxis Emergency Plan or a provider order for the EpiPen. Please note, all medication orders expire at the end of the school year. Medication orders must be renewed at the beginning of each school year (Dated after June 30th)

For students that require an inhaler: 

Please complete and upload the following which may be found under forms: 

● Massachusetts Asthma Action Plan document

● Medication Order Form. Please note, all medication orders expire at the end of the school year. Medication orders must be renewed at the beginning of each school year (Dated after June 30th)

For students that require prescription medication to be given during the school day: 

● Medication Order Form. Please note, all medication orders expire at the end of the school year. Medication orders must be renewed at the beginning of each school year (Dated after June 30th)
