Teacher Recommendation Form

Lakewood Elementary First LEGO League Robotics (3rd Grade)

Applicant’s Name: _____________________________________________________

Dear Teachers,

In writing a recommendation for the above applicant, please keep in mind that the student must meet UIL eligibility. In other words, he or she must be in good academic standing at all times. There are only 5 students total allowed on the team and each student plays a significant role at the “coopertitions.” Therefore, please consider work habits because if the applicant isn’t passing a course at any given time, he is not eligible to attend and this would affect the entire team. Additionally, the judges are constantly evaluating teamwork behaviors and individual character. This is actually a separate judged event. Consequently, the student must exhibit A+ character and must work well with others in order for the team to thrive.

Please use the space below to write your thoughts. When completed, please turn this form into my mailbox placing it in the provided manila file folder.

Thank you for your time!

_______YES, I highly recommend the above applicant.

_______I’m sorry. Based on the criteria, I am unable to recommend at this time.
