Spring Creek Parent Questionnaire

Regarding class placement for 2026-2027 school year

The purpose of this questionnaire is to receive your input as to what type of teacher will best meet the learning needs of your child.  We are NOT asking for specific teacher requests - simply how best to meet your child’s academic and emotional needs. These questionnaires will be viewed by the current grade level teams and be passed on to next year's teacher. Please fill out and return by Wednesday, May 6th

We do realize there are some circumstances where a parent may need to request to have or not have a teacher.

 These reasons include: 

• Having a very negative experience with a teacher previously
• Informing us of a student that your child can not be in the same class with.

 Some non reasons for requesting:  

• You have heard bad things about a teacher
• Your child wants a certain teacher
• You have had a positive experience with a certain teacher
• You want your child to be in a class with their friend
• All of your kids have had this certain teacher

If you have a reason to request please call the office and we will email or mail you a form to fill out.  After we have received the form back you will be contacted by the principal to discuss the matter.
