How to complete this form in Family Access
- Login to your Family Access account through www.gcisd.net. Click on the Skyward icon located at the top of the GCISD website.
Enter your Skyward Family Access Login ID and Password.
- You should see An Online Form is now available to fill out in the middle of your Family Access wall. Click on the link to Fill out Online Form for…
- If you do not see the online form in the center of your wall, select the Online Forms tab on the left sidebar.
- Please fill out the form in its entirety and once it is completed on the form, make your selection from the drop-down and click Complete Step 1 and move to Step 2.
- In step 2, click Submit GT - Parent Permission to Testbutton.
- You’ll then see a confirmation stating you have completed and submitted the form.
