Replace a Lost or Stolen Birth Certificate
To obtain a Florida birth certificate, contact your local health department’s Vital Statistics office (Visit Alachua County Vital Statistics). You’ll need a valid form of photo ID (such as a driver’s license, State ID issued by the DMV, passport, or military ID), and a completed Birth Certificate Request Form (para Español: Solicitud de una Partida de Nacimiento en Florida). To obtain an out-of-state birth certificate, contact your birth state/territory’s Vital Statistics office, accessible here. The current cost (2024) for a copy of a Florida birth certificate is $9. For a list of agencies that might be able to help pay for the cost of the document, please see the “Financial Assistance” section.
Replace a Lost or Stolen Social Security Card
Depending on your situation, you may be able to apply for a replacement card online. Complete this questionnaire to learn if you qualify. Otherwise, stop by the local Social Security Administration office and complete an application. The card is free, but you will need to bring either an original or certified copy (no photocopies) of an unexpired form of identification, such as a U.S. passport, driver’s license, employee ID, school ID, marriage or divorce record, health insurance card (not Medicare), military ID card, adoption record, or life insurance policy form. A form of identification with a photograph is preferred, but the office will generally accept a non-photo document if there is enough identifying information (i.e., name, age, date of birth, parents’ names). Contact the Social Security office to be sure. You will also need to fill out and bring the Application for a Social Security Card (Form SS-5). The Gainesville office is located at 4562 NW 13th St, Gainesville, FL 32609. For support completing this task, call 800-772-1213 (available Monday through Friday, 8AM - 7PM).
Obtain or Replace a Photo I.D.
Florida driver licenses and ID cards expire after eight years. You can acquire a State of Florida photo identification card at your local Division of Highway Safety & Motor Vehicles office (Alachua County offices can be found here.) If the license was stolen, however, and you file a police report, bring a copy of the report to the office, and the fee will be waived. If you are a U.S. citizen, you’ll need to bring a form of primary identification (an original copy of one of the following: U.S. birth certificate, unexpired U.S. passport or passport card, Certificate of Naturalization, or Certificate of Citizenship), proof of Social Security (an original copy of one of the following: Social Security card, W-2 form (not handwritten), paycheck/stub, SSA-1099, or any 1099 (not handwritten)), and proof of residential address. ID requirements will change May 2025. If you are not a U.S. citizen, visit the FLHSMV for more information. Identification cards (original, renewal, and replacement) cost $25, and a typical driver’s license (Class E) costs $48. Apply for Public Assistance (Food Stamps, Medicaid, or TCA)
To receive public assistance funds such as Food Assistance, Temporary Cash Assistance, Medicaid and Refugee Assistance, you may apply at the Florida Department of Children & Families office at 1000 NE 16th Avenue, Bldg. J, Gainesville, FL 32601. Make an appointment online. You can also apply online or call the ACCESS (Automated Community Connection to Economic Self-Sufficiency) hotline at (850) 300-4323. See the Financial Assistance section in this guide for other helpful agencies and resources.
Register to Vote
Click here to visit the Alachua County Supervisor of Elections. To register to vote in Florida, you may apply online, in person, or mail in a paper application (printed copies are available at the Supervisor of Elections at 515 North Main Street, Suite 300 Gainesville, FL 32601). You must be at least 18 years old in order to vote and a state resident. Registration closes on the 29th day prior to an upcoming election and does not re-open until after the election. If you move within the county, Florida Law requires that you notify the Supervisor of Elections of your change of address.
Obtain Police Clearance to Enter a Shelter
Prior to gaining entry into a local emergency or cold-night shelter, you will need to present police clearance to shelter staff (i.e. a “blue card”). This is not a background check; it confirms that you have no outstanding warrants on your record. Obtaining a blue card is a free process completed at the Gainesville Police Department located at 545 NW 8th Ave, Gainesville, FL 32601. You must have a photo I.D. to get this clearance.
Apply for Alachua County Social Services
The Alachua County Department of Social Services offers a number of social services to those who meet eligibility guidelines. These services include health care assistance, medical referrals and prescriptions, and assistance with rent and utilities, and more. To sign up for these services, make an appointment by calling (352) 264-6750 (between Monday and Friday, 8:30AM - 4PM), or emailing socialservices@alachuacounty.us. The office is located at 218 SE 24th Street in Gainesville. Find a Therapist
Here are some questions and tips to help you find a mental health provider:
- Do you have health insurance? If so, you may want to find the providers who are in-network with your insurance to allow for more affordable treatment. Call the phone number on the back of your insurance card or log in to your health insurance portal.
- Do you work somewhere that has EAP benefits (Employee Assistance Plan)? If so, you will usually be provided a number of free sessions. You may also be able to find a provider who will accept your EAP benefits and also be able to use your insurance benefits once EAP stops covering.
- Are you a Santa Fe or UF student? Visit your campus counseling center to explore free mental health supports.
- If you have no health insurance, or minimal financial means to afford therapy, there are a few ways to obtain free or low-cost professional support:
- The Alachua County Crisis Center provides free walk-in emergency counseling or ongoing counseling for individuals, couples, and families who are Alachua County residents.
- Meridian Behavioral Healthcare provides mental health treatment for uninsured individuals in Alachua County.
- OpenPath Collective is a directory that hosts therapists throughout the state who are able to provide therapy for as low as $30/session.
- Some private practices are known to provide free or low-cost therapy. Just call and ask!
