Trauma C-section

Indications:

• Maternal distress
• fetal distress:

• FHR < 100
• prolonged decelerations for > 60sec
• recurrent prolonged decelerations

• fetal viability:

• > 26 week
• presence of fetal heart tones

•  has 75% survival
• independent of maternal distress or injury severity score
• Success is dependent on time between maternal death and delivery

• <5min - excellent
• 5-10min - good
• 10-15min - fair
• 15-20min - poor
• 20-25min - unlikely

• Procedure:

            1. establish viability

            2. hold CPR

            3. midline incision to uterus

            4. remove fetus, clamp cord, hand off infant

            5. remove placenta

            6. coninue CPR
