1. Grant of Rights 

I, _______________________________ hereby grant Solebury Soccer Club (hereinafter referred to as the "Recipient") the right to use and publicly display my child’s image, likeness, voice, and any other identifying aspects of my child (collectively, "Likeness") in any media, including but not limited to social media platforms, websites, advertisements, and promotional materials.

2. Scope of Use 

The Recipient may use the Likeness in connection with the promotion, marketing, and other related activities of Solebury Soccer without further compensation, consideration, or notice to me. I understand and agree that the Likeness may be used in whole or in part, and may be combined with other images, text, and graphics.

3. Waiver of Rights 

I waive any right to inspect or approve the final product(s) in which my child’s Likeness appears. I also waive any claim for damages or remuneration associated with the use of the Likeness, including any claims related to defamation, invasion of privacy, or violation of any other rights.

4. Release of Liability 

I release, discharge, and hold harmless Solebury Soccer and its agents, employees, representatives, and assigns from any claims, demands, or causes of action that I may have arising out of or in connection with the use of my Likeness as described above.

5. Acknowledgment and Understanding 

I acknowledge that I have read and fully understand the terms of this waiver. I certify that I am at least 18 years of age (or have obtained the necessary consent from a parent or legal guardian) and have the legal right to grant the permissions stated herein.
