Oregon State University Gaming Club: Event Waiver Form

Section I: Definitions

For the purposes of this document the following terms shall be used:

Section II: Agreements and releases

To be allowed to participate in any way in the event, the undersigned:

  1. Agrees that they are of the age of majority or are otherwise legally able to agree to the terms of this release. If not able to do so then a parent, legal guardian, or other individual that is legally able to enter into the terms of this release on behalf of the undersigned will have to sign this form.

  1. Acknowledges and understands that each attendee will be participating in activities and contests that may involve risk of serious injury to their person and/or personal devices. These risks include, but are not limited to viruses, power failure, network failure, and equipment damage/theft. These risks may result from personal actions, inactions, or negligence of the attendee or others at the event.

  1. Assumes all foregoing risks and accepts personal responsibility of any damages or losses.

  1. Acknowledges that the attendee will not be covered by any insurance for their equipment or person from any of the releasees, and agrees that the attendee is solely responsible for any and all costs that may result in any way from participation in the event.

  1. Agrees to waive, and discharge, the releasees from ANY AND ALL LIABILITY to each of attendee, his or her heirs and next of kin, for any and all claims, demands, losses, or damages on account of injury including death or damage to property caused, or alleged to be cause in whole or in part by the negligence of the releasees. Further the releasees SHALL NOT BE LIABLE FOR PUNITIVE, INCIDENTAL, CONSEQUENTIAL, EMOTIONAL, OR SPECIAL DAMAGES WHETHER OR NOT SUCH DAMAGES COULD HAVE BEEN FORESEEN.

  1. Agrees that criminal activity of any kind, including but not limited to; theft, vandalism, drug violations, assault, and/or harassment, will not be tolerated and will be reported to the appropriate authorities immediately.

  1. Agrees that the attendee may come in contact with subject matter that may only be appropriate for mature individuals, this may include but is not limited vulgar language, violence, and nudity.

  1. Releases, waives, and discharges all rights to photographs, video recordings, and/or audio recordings taken at the event. They understand that their picture and likeness may be used in publications and on websites in the future for promotional and informational purposes. They also agree to grant rights to the releasees to use without compensation any photographs, video recordings, or audio recordings taken at the event by the attendee, should the releasees come into possession of aforementioned items.

  1. This agreement shall be governed by the any and all applicable law governing Corvallis OR USA.

  1. Has read and agrees to abide by all of the rules for the event, as well as any and all instructions from  agents, officers, and employees of The Oregon State University Gaming Club, the facility, law enforcement agencies, emergency medical and fire agencies.

I have read the above waiver and release, understand that I have given up substantial rights by signing it, and sign it voluntarily.

Section IV: Signature of Parent or legal guardian (only required if the attendee is under 18)

Parents/Guardians please check one of the following:

I certify that I am the parent, legal guardian, or otherwise legally able to enter into agreements on the behalf of of the attendee named and signed above. I will take full responsibility for the actions of the minor attendee whether or not I am also attending the event. I certify that I have read the above waiver and release, that I understand that substantial rights are given up by signing it and sign it voluntarily.

OSU Gaming Club Rules for all events

  1. There is to be absolutely no Alcohol, drugs, or controlled substances of any kind in the event or on the premise.
  2. The use of tobacco within the event is prohibited.
  3. Weapons, including but not limited to firearms, swords, knives, pellet guns, air guns, clubs, etc. are not allowed in the event.
  4. Fighting of any kind including but not limited to punching, kicking, brawling, scratching, hair-pulling, and other forms of physical altercation are not allowed in the event. Fights may be broken up by force if necessary, and law enforcement may be called.
  5. There will be no refunds once you have paid. Be sure you are able to attend before you pay.
  6. Attendees must present proof of registration each and every time they enter the event. To receive proof of registration you must have paid, provided a legal form of photo identification at the entrance, and turn in completed, signed copy of the waiver form. Anyone not able to provide a legal form of photo identification will be assumed to be under the age of 16 and subject to the rules regarding age below.
  7. Attendees under the age of 18 must have their waiver signed by a parent and/or legal guardian.
  8. Attendees under the age of 16 will not be admitted without the supervision of a paying parent, legal guardian, or family member. Attendees under the age of 16 may not be left in the event unsupervised at any time.
  9. Each attendee must provide a signed copy of the waiver releasing the event, the event organisers, the venue, sponsors, anyone acting on authority of these entities, and/or other attendees of all liability for all damages or harm to their persons or property.
  10. There are to be no deliberate or repeated actions that cause disruptions to the network.
  11. Attendees are not allowed to tamper or otherwise interact with equipment not belonging to them without prior permission from the owner of said equipment.
  12. Attendees are not allowed to run DNS, or DHCP servers of any kind. Including but not limited to routers, network connection sharing, etc.
  13. Attendees are not allowed to host any servers of any kind allowing connections from outside the event. (A local IP range may be made available upon request)
  14. Participation in illegal file sharing and/or copyright infringement will not be allowed.
  15. Torrenting is not allowed.
  16. Attendees should make an effort in ensure they have downloaded and/or updated all applications, games, or other software before arriving.
  17. Sleeping in the event is not allowed, those found sleeping will be awaken.
  18. Disruptive or offensive behavior is not allowed. Report violations to event staff.
  19. Good hygiene of the attendees and their space is required, those not in compliance will be required to leave and remedy the issues or clean up their space. Report violations to event staff.
  20. Attendees will be responsible to pay for damages or replacement costs of any event equipment they break, damage, or interfere with.
  21. Distribution of pornography is not allowed. Distribution of pornography to minors, possession of pornography by minors, and possesion of pronography of minors are illegal and will be reported.
  22. Attendees will have 3 linear feet of table space per purchased seat  to setup their equipment, if you want to use additional space you must purchase an extra ticket.
  23. Attendees will have access to 650W of power per seat.
  24. Event staff has the final say on all matters, and may add to, subtract from, and/or enforce these rules to the extent permitted by applicable law.
